I Recently Noticed That I Have Been Unconsciously Rolling My

default
Posted on Thu, 3 Oct 2019
Question: I recently noticed that i have been unconsciously rolling my left thumb and forefinger together in a circular pattern. I know from classes I took in neurology that it is called "pin-rolling." I have been doing it for more than a year but I am doing it more often now.

I know it is a sign of Parkinson's disease (which my father has) but I'm wondering if it could be related to the medications I take (Lamictal, celexa, trazodone, low dose klonopin...occasionally ritalin or modafinil). When I take celexa I frequently wake up with significant tremors, and racing, fractured, uncomfortable thoughts. A small amount of coffee or tea calms them and I can sometimes return to sleep.

Brief Answer:
Yes, it could be drug induced parkinsonism.

Detailed Answer:
Hi sir,
Welcome to ask a doctor service,

I have gone through your query in detail and can certainly understand your concern towards your symptoms.

Yes, definitely it could be drug induced parkinsonism as it may be caused due to antiepileptics like lamotrigine that you are on.

So, I would like to advise you to taper the dose of lamotrigine to the required one and observe the intensity of tremors.

You can discuss with your doctor regarding the tapering of this antiepileptic and considering another one depending on the requirement.

Hope this information would help you.
Thank you.

Thank you. I only take 100mg of brand name lamictal. But I have taken it since 2005 at varying doses. Higher doses result in sleep and cognitive disturbances, headaches.

I have also been taking klonopin .25-.5 mgs daily since 2011. I have had numerous brief trials with gabapentin, trileptal, several second generation neuroleptics over the years, and most recently almost two years on MAOIs (which is when I noticed it first). I have also been on anti-HIV drugs since 2005.

My father has also been taking psych meds most of his life and believes seizure-like activity began after years of taking lamictal also. Mine mostly occur before-during sleep or at waking. Sleep neurologist found nothing.

I THINK I only do this on my left side, where I've had various parasthesias over the years. I also have cervical nerve impingement on the left side.

Unfortunately, Lamictal is one of the only consistently helpful parts of my medication regimen.

Is there a way to test for this without stopping lamictal and/or rule out other issues? An MRI?

Thank you

Brief Answer:
It can be replaced by any other mood stabilizer.

Detailed Answer:
Hi sir,
Welcome back,

I have gone through your follow up query and can certainly understand your concern towards the drug lamictal

But, it is advisable to compensate lamictal with any other mood stabilizer to know whether the tremors are due to lamictal or not.

This is the only way to understand the cause for tremors in your case.

So, nothing to worry , lamictal can be replaced by another mood stabilizer.

You can discuss with your doctor regarding the replacement of mood stabilizer.

Hope this information would help you.
Thank you.
