Question: I recently fought a bout of insomnia. After feeling that I had gotten a significant amount of sleep I still feel dizzy and off balance, even after a few nights of restful sleep. What might be the cause of this?

Detailed Answer:
If you are suffering from dizziness and dysequilibrium which is something that only recently has come on AND is not accompanied by severe nausea, vomiting, tinnitus (ringing in the ears), vertigo or spinning nor accompanied by muscle weakness on a particular side of your body then, you may be experiencing NONCRITICAL symptoms of a labrynthitis which is likely VIRAL in nature and will typically subside in a period of days to weeks. If therre is an earache associated, fever, or any other constitutional symptoms suggestive of a bacterial infection then, you may be suffering from an otitis media and this may require antibiotic therapy.

If there are no symptoms of an infection in the head such as an otitis or labrynthitis then, the problem could be medication induced. You say that you use a CORTISOL MANAGER but such a product is not likely something which is prescribed (at least not by an MD/DO physician) and therefore, there may be ingredients or elements in this product causing you problems so I might suggest you stop use to see how you feel. Otherwise, if you're on other medications such as those for blood pressure, water pills, antidepressants, etc. then, any of these medications (especially if recently changed in dose or if brand new) could be responsible for your symptoms.

It is less likely that you should have a stroke since such a thing would've very likely been dramatic and you wouldn't just be dizzy but rather you may not be able to properly stand or walk and you'd likely be feeling quite queasy and sick to your stomach.

Therefore, at this point in time with the very little amount of information provided I think the most likely diagnosis is that of a viral syndrome which could be primarily affecting the inner ear apparatus and the mainstay of treatment for that type of problem is good hydration, not skipping meals, and also NOT BECOMING inactive unless the dizziness is so aggressive that you have trouble standing or moving about. If that is the case then, you need to see a doctor and have a complete evaluation.

Other organic sorts of problems that could be afoot involve looking at THYROID HORMONES and certainly ADRENAL HORMONES and catecholamines could be an issue especially if there are any problems with blood pressure etc. Also, there is something called ORTHOSTASIS which is blood pressure actually drops to levels that are low enough to cause dizziness or lightheadedness so a simple test that can be performed by a doctor is called a modified AMINOFF TEST OF ORTHOSTASIS where a series of blood pressures and heart rates are taken over a 10 minute period in different positions of lying, standing, and prolonged standing. Readings are then, interpreted on the basis of how a patient feels and if excessively affected by posture or out of proportion to what would be considered physiological then, the patient may be said to have orthostatic hypotension....which then, must invoke a workup since there are a number of causes. But I don't think things would necessarily get that complicated in your case. Start with simple and common things....and then, we move to more complex and rare items on the menu, Make Sense?
