I Have The Urge To Urinate Right After Urinating And

Question: I have the urge to urinate right after urinating and some slight irritation in my urethra. I have tested negative for all bacterial infections (chlamydia, gonorrhea, trich, mycoplasma) and no bacteria was found in my urine culture. After taking a week of doxycycline, the symptoms became better but not fully eradicated. I then took three days of azmithromycin and cipro together but no improvement. I have no pain or discharge, only the urge to urinate right after urinating.

I was diagnosed with NGU some months before and the doctor prescribed doxycyline. The problem went away after taking the medication but now doxycycline seems like it is not working. I have genital hsv-1 but have not had an outbreak since 2018. how do i solve this problem?

I have gone through your query and here is my opinion.

It is very likely that your symptoms of urge to urinate after urinating is due to an overactive bladder(OAB).
Recurrent infection is a common cause of Overactive bladder,in addition to weak bladder muscles and neurological disorders.

I think that your NGU and HSV 1 have been adequately treated because you do not have any symptoms like
discharge,pus,eruptions,swelling,fever or pain.

You had a proper antibiotic course so it is very unlikely that you have a residual infection.
Your urine culture has validated this.
In such a scenario,indiscriminate use of antibiotics should be avoided as later on,it may lead to emergence of resistance.

In my opinion,there is a need of further evaluation in the form of ultrasound scan for residual urine,cystourethrogram and pressure studies.

Such patients may need to learn biofeedback, pelvic floor exercises and may need drug treatment.
