I Have Had A Swollen And Painful Knee For Over

default
Posted on Wed, 5 Feb 2020
Question: I have had a swollen and painful knee for over 5 weeks; it started to hurt and then became very swollen without any reason, i.e. no injury was sustained. After 2 weeks of rest and NSAID I went to see a physio, after a good look and prod he recommended that I needed to see a GP because there was no logical reason for the swelling and that I would need Blood tests and an X-ray. The GP agreed with the Physio and off I went for those Bloods and Xray. The results have come back the x-ray shows what would be considered normal wear and tear for a 50 yr old male. The bloods for CRP came back at 28 and the ESR also at 28. My understanding is that the reference range for CRP is 0-5 and 1-10 for ESR. I contacted my Dr’s Surgery and ask for the results of the Blood test and the opportunity to discuss with the GP, to which I received a reply that “my blood test results were normal and therefore no further action is required”. My Physio definitely disagrees with this statement from the Dr’s Surgery.
So, the question is, are my blood results normal with a CRP of 28 and ESR of 28, is the Dr’s Surgery correct to say no further action is required?
The swelling on my knee has reduced over the weeks to about 15% of the initial swelling, I still have pain especially in the morning and after long periods sat at my desk ( I work in IT) and it does sometimes feel warm to touch, but it is not hot nor red.

Brief Answer:
Need to rule out Gout

Detailed Answer:
Hi there

Thanks for the query

I have gone through your history and viewed the attached photograph of your knees as well. There is swelling over the knee which can be attributed to possible inflammatory arthritis.

Since CRP is high it would be best to get your knee aspirated and tested for crystals and Gram staining with cultures to rule out Gout and infection respectively. Also Pre patella bursitis needs to be ruled out. Hence I would disagree with the fact that your GP does not want to take any further action.

I hope I have answered your query. I will be available to answer your follow up queries.
