I Have Been Having Nasal Drip For A Month. It

Question: I have been having nasal drip for a month. It builds up in my throat and does not let me sleep. I took Claritin, then Zyrtec. I have been quarantined since March 14th to protect my family and myself from Covid 19.
I was working in the house cleaning the dust, used strong cleaners, and I have also been working in the backyard. I then started with nasal congestion and then came the nasal post dripping. I started taking Tussin DM because I had a little allergic cough. At night I took 50mg Benadryl and I was able to sleep all night, but if I still felt dripping.
The dripping continued to get worse and a virtual online doctor prescribed Altipress B every 4 hours, one Zyrtec a day, and Fluticasone propionate nasal spray before bed. The dripping continued and my throat was sore.
Then another virtual online doctor sent me a 500mg Augmentin antibiotic, Fluticasone nasal spray, and another Nasal Spray (Azeslastine HCL). Both nasal sprays were prescribed for the morning and at night. There was sore throat relief, but not nasal post dripping continued.
On the third day of treatment I felt so much accumulation of mucosa in my throat that I added Altipress B and Zyrtec at night, In addition to Albuterol therapies every 4 to 6 hours (whenever I felt chest tightness).
For two days I felt better and stopped the Albuterol therapy on the third day.
Since I also had some bloating and reflux, I started changing the diet, dining earlier, and (as per doctor’s indications) taking Protonix 40mg in the morning and Pepcid 20mg in the evenings. My stomach felt better.
I continued treatment with Altipress B but the nasal post dripping returned again. I consulted with a third virtual online doctor and she agreed with the previous doctors that I had upper respiratory tract allergy. She instructed me to continue taking the Altipress B, Zyrtec and only Fluticasone propionate (as the Azestaline HCL could increase the sore throat).
I followed the indications but in 24 hours the congestion increased (now my ears were also congested). The next day I stopped taking the Altipress B and started taking Claritin D 12 hours (as per pharmacist’s recommendations). Also continued with the Fluticasone nasal spray and Tussin DM (expectorant and cough relief). I spent two days quite well but at the night of the second day the post dripping returned gradually. The dripping increased on the morning of the next day.
I do not know what to do anymore. I want to clarify that I've barely had a cough but when I get occasionally it's an allergic cough. Before the ear episode I did not feel the congested nose, I could breathe well, and I haven't had runny nose; all the drip is internal. Any mucus that I have spitted is clear. In addition, I have made home remedies, such as warm gargles of salt water and I ginger tea (with lemon and honey).
I have been looking in my country (Puerto Rico) for a virtual online otolaryngologist without success; only family doctors are available online. I want to clarify that I have no fever or tiredness. I only had a brief short of breath (on two occasions) and then was fine. I believe it could possibly have been due to the same congestion. In addition, I am the only one that has been sick in my home. Both my husband and my children are fine.
I have done everything online and everything is brought to my house since March 14th. No one in my house (including my husband and children) had any contact with anyone outside and no one has left the house since we started the quarantine. Due to all this situation I have some anxiety at night, and I can’t sleep.

You seem to be suffering from the following conditions:
1. Allergic Rhinosinusitis
2. Laryngopharyngeal reflux secondary to the acid reflux disease
3. Eustachian tube salpingitis

Do not worry! With the appropriate medications and the home remedies, you should be getting better.

1. Continue Fluticasone nasal spray, 1 puff in each nostril 2 times a day. This is beneficial in reducing the nasal allergy, postnasal drip, and opening up of the Eustachian tube. You need to use this for at least 3-4 months continuously.

2. Continue with Pantoprazole 40mg, 1 tab in the morning before the breakfast. Pantoprazole reduces acid-reflux, thus reducing the post nasal drip and the soreness in the throat. This tablet has to be continued for at least a month.

3. Use nasal decongestants such as Xylometazoline or oxymetazoline,1 puff in each nostril 3 times a day x 5 days, followed by steam inhalation after 10minutes. This spray helps in opening up the nostrils, sinuses, and the blocked Eustachian tube.

4. A mucolytic such as bromhexine or ambroxol (both available over the counter) can help in reducing the mucus accumulation in the throat.

5. Drink frequent sips of warm water.

6. Avoid sleeping in front of the A/C or directly under the fan.

7. Change the pillow cover every night before you hit the bed.

8. Avoid coffee, smoking, chilled food and beverages, and fizzy drinks.

9. Cover your nose with a mask or a clean cloth when exposed to the dust or smoke.

10. Give a gap of at least 1 hour after your dinner before you go to bed.

With the above medications and the home remedies, you should get better. Avoid anxiety, this can worsen your acid reflux.

1. The headache is not because of your medications.

2. If it is Bromhexine it’s 8mg and if it’s Ambroxol then 15mg or 30mg would suffice.

3. It’s always mentioned as 3 days just to prevent the misuse by the patients. Trust me, you can take it safely for a period of 5 days. Once you spray this, you should do steam inhalation after 10minutes. This helps in clearing the mucus and opening up the sinuses and the Eustachian tube.

But, I would not recommend you to use it more than 3 times a day, because, the spray would irritate the mucosa of the nose and also cause dryness. Hence, you need to drink frequent sips of warm water.

4. Continue with the medications for the acid reflux, that would make you feel much better and the mucus secretions, chest and throat discomfort would subside.

5. Do not worry! You would definitely overcome the present issues very soon. Please stick on to the medications I mentioned earlier.
