Posted on Fri, 13 May 2016
Question: I have been having a pain in my lower abdomen, left side with frequent urination and some difficulty urinating. In the past I have experienced kidney stones and also urethral strictures. I have not had a problem with the strictures since I had surgery at HUP 12 years ago.

Brief Answer:
Possibility of stone disease recurring

Detailed Answer:
Hi, Welcome to HCM,

The symptoms that you have described are collectively called strangury, which is a feature of both kidney stone disease and urinary infection. If you have not been troubled by stricture of urethra for over 12 years, it is unlikely to be the cause at present.

A good way of knowing what we are dealing with is by getting an ultrasound scan of KUB, kidneys and urinary bladder, done with a full bladder, followed by an estimate of post void urine.

Urine can be sent for analysis, and if pus cells are numerous, urine culture and sensitivity should be ideally done. If an ultrasound scan is suspicious of stones due to presence of swelling of kidneys or if a stone has been noted, an X-ray KUB should be done, followed if necessary by CT scan of KUB.

Some kind of empirical treatment can be started, and you could try Alkaline Citra syrup thrice daily diluted with water and taken after meals.

Tab Flomax or Tamsulosin once daily is useful on helping you void with minimal discomfort, and may help passage of small stones, if any, from the lower ureter.

If urine shows pus cells, after giving urine for culture and sensitivity, you can be started empirically on Urinary antibiotics Tablets such as Nitrofurantoin 100mg twice daily till the culture report is ready.

You could also try and improve your fluid intake as well.

Hope I have helped you.
Regards.
