Acne is a chronic skin problem so new ones can keep coming up upto a certain age. You need to first take effective therapy to control acne activity and then continue with a maintenance regimen. You seem to have inflammatory acne i.e red, painful bumps. Kindly upload a photo of acne, so that I am better able to help.
I would suggest you to take an oral antiacne antibiotic e.g either Doxycycline 100mg or Minocycline 100mg for a few weeks.
In addition I suggest you to use adapalene + clindamycin combination gel at night e.g Deriva CMS gel on acne only. For daytime I suggest you to use Benzoyl peroxide gel e.g Microbenz 3.5% gel on your active acne.
Besides that use a mild cleanser for face wash e.g Cetaphil gentle cleansing lotion.
Stop all cosmetics like moisturizers and sunscreens. Avoid hair oil.

Just to be clear I'm talking about the scalp acne, I did consult a dermatologist couple of months ago of which I'm uploading the prescription, for the time being the acne subsided but after I stopped taking medications the acne came back, its frustrating.
And yes when I have the acne I have a considerable hair fall as well.
Please suggest me a cure to this, I would like to end this cycle for once and all.

I suggest you to take Cap. Doxycycline 100mg for a few weeks. In addition I suggest you to use Clindac A 1% solution for your scalp acne. You may continue with the shampoo. Shampoo daily. Avoid hair oil. You may rather use ammonium lactate lotion (Lacsoft lotion) instead of hair oil on wet hair. Take care.
And from when Can I start to use oil, hair cream on my hair.
Lastly, so the lotion you’re suggesting is to be applied to wet hair??

Hi.
I suggest Cap. Doxycycline 100mg once every night after meals for 4 weeks. Use the solution on the affected scalp areas on air dry scalp, twice daily. Not on wet hair. Avoid hair oil if you keep developing recurrent scalp acne. Use Lacoft lotion instead. It would serve the same purpose as hair oil but without causing acne. You may use hair styling gels if you want but hair creams are usually greasy. Take care
