Question: I did an ekg recently and the report came back that I have inferolateral subendocardial ischemia. Four years ago I went into sudden cardiac arrest with v fib. Can you explain my ekg

In such case, performing a coronary angiography would be needed in order to determine the affected coronary arteries.
Have you performing any stress tests ( cardiac exercise stress tests or stress echo) during the past years as you have been suffering from cardiac arrhythmia? Have you had any episodes of chest pain or shortness of breath during physical exertion? Do you have any previous ECG images?

Below are the results of my ECG. I'd also like to give you a little background as to my physical conditioning. Prior to my Sudden Cardiac Arrest, I exercised daily, either road cycling (9 months a year) or skiing (3 months a year). I was very fortunate and did not have any mental or physical decline from my SCA. I continue to road bike (25-40 miles) daily, use a Peloton exercise bike, or ski every day. I have had no decline in any of these activities. I am a very active 75 yr old. In essence, I give myself a stress test every day. I have had a history of long QT. I'm told the V-Fib event I had was caused by a piece of plack breaking off and sending my heart into V-Fib. I had a stress 2 years ago and was told everything was fine with the exception of my left ventricle being thicker. I'm not sure if this information is helpful, but I wanted to give you a little background on my physical health.

I failed to mention that after my SCA I had a pacemaker and ICD installed in my chest. Seems a very normal part of my body and forgot to inform you.

It seems that you have hypertrophic cardiac phenotype pattern. Your ECG patterns are compatible with such findings. Whether it is hypertrophic cardiomyopathy or similar phenocopies it may be clarified by means of additional cardiac tests (besides cardiac ultrasound), like cardiac magnetic resonance, cardiac PET, etc.. I don't know if you have performed any before. Heart failure, and dangerous cardiac arrhythmias and conductance disturbances are the main potential complications.
Anyways, as a pacemaker-ICD is implanted you are more safe in this regard. Your ECG features shows a borderline QTc interval as well.
I recommend following you active lifestyle and avoiding very strenuous physical activity. Periodic cardiac imaging tests are advisable.
