Question: I am really worried i will die. I haven't been feeling myself. I've been feeling very tired and have pains in my sides and lower back. I have had an endoscopy and have a sliding hiatal hernia of 4cm. I had kidney, liver, pancreas and full blood count blood tests and these are normal. I had a heart ultrasound and this was normal. I am awaiting a colonoscopy which I'm having tomorrow and an ultrasound of my abdomen this week on Thursday. I do get tired after a bowel movement in the morning. I'm worried i will die due to how i feel.

It is good you already had some tests and that they are normal. There are a few more tests to consider that I did not see them mentioned:
- TSH and FT4 to check your thyroid function as thyroid problems may sometimes cause a patient to be tired and anxious
- I did not see it mentioned, probably you have had it done but a test to check the fasting glucose or HbA1C is important to rule out Diabetes
- Also, a test for mononucleosis (infection from EBV), and ANA, RF, ERS to see for autoimmune conditions may be advisable.
- If you also have bowel movement changes (diarrhea) or abdominal pain, a blood test to check for gluten intolerance may be needed. The colonoscopy may also see changes related to gluten intolerance if present.

If all the tests and examinations come out normal, then anxiety can be the cause of the symptoms you experience. Anxiety may cause patients to fear they are going to die and may also cause tiredness and in some cases patients may have pain in certain areas of the body while all the tests come out normal.

In this case, SSRI drugs (such as sertraline or citalopram) and psychotherapy may be helpful and also making sure you have a healthy lifestyle (no alcohol, no smoking, no more than 1-2 coffees a day, being involved in exercises and keeping yourself busy with things you like but making sure you are not overloaded but rest as needed).
