I Am Male 50 Years, 6feet Tall, 100kg. I Feel
Question:
I am male 50 years, 6feet tall, 100kg. I feel breathless on exertion (even simple walking, bending, etc) and fatigued. I also face night-time breathing issues (OSA). I am not able to use CPAP. My SPO2 levels measure with home pulse oximeter is 94-95.
I also suffer from frequent chest pains. Recently I was hospitalised with chest pain (with elevated troponin). Prior to admission I was required to have a screening CT chest to rule out COVID. The CT was normal for COVID, but had a couple of other findings which concern me.
I also had a PFT last year which shows moderate issues.
I attach both these reports. Please study them and let me know what the problems are. Are they serious? Do i need to do any further tests / treatment for these.
Also, my cardiologist found only smaller blocks in my angio, (along with a myocardial bridge) and he feels those should not be the cause of my breathlessness and chest pain. Can these lung issues be the cause of chest pain / breathlessness.

1. What is subpleural atelectatic changes in both lungs? Is this temporary or permanent? Is it serious?
2. What are medistinal lymphnodes? Are they cancerous? What are subpleural node in apico-posterior segment of left upper lobe? Cancerous?
3. Can there be any cause other than OSA for the above findings?
4. Do I need to investigate / treat all these any further or is using CPAP the only thing to do?
5. Can this cause chest pain / fatigue?

Thank you, doctor. No, I have not yet seen a pulmonologist after this. This was done as covid screening to get admitted to hospital for chest pain. Acs Nstemi.
Due to covid, is it risky to visit pulmonologist and do bronchoscopy and other tests?
Am I at higher risk for covid due to osa and above conditions?
Please advise me whether it is better to wait for few months till covid position improves or is it better to visit pulmonologist right away. I stay in XXXXXXX
