Question: I am having allot of pain after bowel movement the doctor said its either a fissure or hemorrhoid I was prescribed two different creams nifedipine and diltiazem. which one should I use, the pain is quite severe and lasts 5-6 hours after a bowel for 3 months now. do you recommend anything else or maybe something to numb the pain.
I had a lower colon scan and they said it was clear but might be a small tear or small hemorrhoid. so what do I treat?

You are having symptoms of anal fissure.
Nifedipine and diltiazem relieve the anal sphincter spasm. But they do not relieve pain.
For complete relief, you need to do the following.
1. Sitz bath for 10 minutes twice a day.
2. Apply topical nifedpine or topical diltiazem twice a day at the anal verge.
3. Apply topical lignocaine twice a day at the anal verge (This relieves pain)
4. Stool softeners (Hard stool causes fissure)
5. High fiber diet and lot of fluids.

Should i continue with metamucil and stool softeners and is lignocaine and lidocaine the same and do i need a prescription. Should i apply vaseline prior to having a bowel movement to help with the pain

1. Yes you need to continue taking metamucil and stool softeners.
2. Yes, lignocaine and lidocaine are the same. No need for any prescription.
