Question: I am a 33-year-old male, 195lbs, 5'9. Asian in ethnicity. Non-smoker, non-drug user and very casual drinker.
Currently on Naltrexone (1.5mg) and custom pain topical compound of gabapentin, ketamine and ketoprofen.

Can a doctor look at my Axial CT slices and tell me if I have any renal vein compression (nutcracker syndrome, SMA, retroaortic left renal vein, etc.)? Please note, these CTs are 6 months old. I am currently seeing a great doctor, but would like to use the power of the internet to supplement my investigation.

I would like to explore possible venous related issues behind my symptoms (described below), as everything else has been normal so far and I still don't know the cause of my chronic pain.


I have a 10-month history of left flank pain (sometimes b-lateral), lower left abdominal pain, upper abdominal ache and nausea, feeling of pelvic tightness and congestion (wearing tight pants or car rides can exacerbate my symptoms) and light headedness when standing and walking. Exercising often exacerbates the light-headedness and pelvic flair up.

Sleeping on my side has also become impossible as this will cause my pelvic and flank pain to flare up. Sitting causes me to get a feeling of high blood pressure while standing makes me lightheaded. Pelvic and flank pains are exacerbated when sitting for extended periods of time.

I also have pain after urination and ejaculation (a strange burning sensation from within my urethra) and intermittent left testicular ache. Have been prescribed gabapentin and naltrexone, which lowered my pelvic pain but did not do much for my flank pains.

Pelvic, abdominal and testicular ultrasound have been normal. Organs are normal on CT/ nothing of note. Blood work and urine have so far been normal. Colonoscopy normal. C-React normal.

Things of note: Have had a varicocele on my left testicle embolized 20 years ago when I was 12.

I would like to investigate possible renal vein or venous issues as well as other issues that are harder to discern on medical imaging (endocrinological issues?).

Hi Doctor,

Thank you for the reply. Is it possible to tell from a quick glance at the images I provided on the PDF and the website? The reason is my DiCom is a massive file (as you know) but also has my doctor's clinical info as well as my personal info on it and I am not quite comfortable sharing it on the internet.

Is it possible for me to upload additional images you might need in pdf format with my details cut out? Please let me know and I can upload them for you.

I am merely interested in what you might see for my kidneys and the renal veins/ arteries in that area.

On axial images does not look nutcracker, please send sagittal images

I respect your privacy and personal information.

From the pictures uploaded it does not look like typical nutcracker where the left renal vein is completely compressed by the SMA. However I would surely like to know by doing image reconstructions, if there is any mild compression on the left renal vein.

Can I request you to please try and make sagittal or 3D images using an image viewing software like RadiAnt and sharing the required images if possible.
