Question: I am 37 weeks pregnant. I was induced with my first through the emergency room because of rapidly rising blood pressure that got worse for about a week after her birth. My blood pressure is rising again. Do I have any (decent) options with this second birth to avoid the chance of post-partum hospitalization? I started medication with my first when I was admitted to the ER.

The rise in blood pressure at 37 weeks is risky and can cause decreased liquor and fetal distress at the time of delivery. It depends on your blood pressure readings. In case the blood pressure is more than 130/90 mm hg then it is best to consult your gynecologist and plan for induction of labor. Otherwise get a few blood tests and non stress test done to rule out any fetal distress. Early delivery can decrease the chances of post partum hospitalization.

For more information about my case, I have a blood pressure monitor at home that I have been using for months, as I wanted to avoid the same thing that happened with the first. Especially since my blood pressure continued to rise post-partum to the extent that I had to be rehospitalized after my daughter was discharged.

My blood pressure readings have been around 135s/95s for about a week with the peak being 150/109. After this reading, I went to the emergency room but my blood pressure had decreased back to 135/95. There are no pre-eclampsia indicators besides high blood pressure (all labs have come back normal). I received an ultrasound yesterday with baby being marked as healthy and no signs of distress except for when she fell asleep during the heart monitoring.

Given my medical history (mainly that birth did not immediately lead to reduced blood pressure), when would be a reasonable time to induce? Or what questions should I be asking my Obstetrician? I have an appointment with her today to discuss but I feel I am not communicating how serious the situation was to my health before.
The objectives for my appointment today are:
-Learn at what BP readings i should be notifying her and at what readings I should go straight to emergency
-Learn a basic plan she could have related to my blood pressure immediately, at 38 weeks, and at 39 weeks (regarding waiting for natural labor vs induction and if induction, what our options are).
My main concern is that I want to give birth BEFORE my blood pressure gets dangerously high, as my blood pressure continued to rise for 1-2 weeks after my first was born (I am told this is an uncommon thing).
Any advice you can give about what questions to ask and what might be some recommendations would be greatly appreciated!

As the pregnancy advances, the blood pressure levels increase further. Your blood pressure is already high and it is best to consider for induction so that it is easier to control it post partum. The levels of blood pressure are difficult to predict before the delivery.
You should consider going for a non stress test to be very sure that blood pressure has not affected the baby.
Your priority should be to consider induction after 37 weeks if possible so that it is easier to control the post partum complications.
