How To Rule Out Metastasis Of Renal Cell Carcinoma?

Question: I have a very rare RCC TFE3 translocation gene carcinoma left kidney, its been removed 15 dec 2015 but i had a chest ct 30 nov 2015 showing cyst in both the liver and the rt kidney, as well as enlarged rt hilar lymph node.
Should i be concerned that the cancer has spread?
My dr refused to beleive it was cancer even tho the biopsy done in oct said it was.
The day of surgery he assured me it was an angiomyolipoma. He said that theres was a "high fat content" Well 5 days later we have the pathology and he says we can wait 4 months and have another scan done to see if there are any changes.

My main questions here :
1) do i have reason to believe that the cancer has spread given the recent scans?
2) is there any test that can be done now that are fairly painless to see if the RCC has infact spread?

I'm 41 years old and have a 7 y/o daughter ans from what i understand metistatic RCC is very hard if not impossible to cure so to me waiting not the answer.

Oh also theres is an area in the left kidney that is a "shadow" 1 cm mid pole left kidney probabal fatty tissue.

Please advise

As an Urologist, i fully understand and share your concern.
As you've rightly mentioned, to give you proper advice,i'll need following reports :
1. CT scan reports.
2. biopsy report.
3. blood and urine reports.
4. Chest X-ray/ CT scan report.
After seeing these reports, i shall be able to give you a proper advice, regarding further treatment.
Definitely this isn't an Angiomyolipoma.(AML).

It's only a localised cancer. No spread seen on CT scan.

Detailed Answer:
Hi XXXXXXX
Having gone through your reports, now i'm in a better position to advise you further.
1. Only part of your left kidney was removed, which showed localised cancer.
2. Rest of the kidney is intact and will need follow-up CT scans at periodic intervals.
3. The CT scan chest, shows some changes of your past smoking habit.
4. The cysts seen in liver and kidney, are normally seen in many people.
It doesn't appear to be cancerous.
A follow-up CT scan is mandatory, at 4-6 months interval, to check for any changes in the area of the remaining part of left kidney and the right kidney.
AML( Angiomyolipoma), was suspected correctly, due to high fat content in the tumor area. It does mimic a RCC cancer very closely.
So be relaxed, that you had only a localised tumor,but be on regular follow-up.

So the small cyst developing in the right kidney in the upper pole (same area as other kidney) are not a cancer growing
And the enlarged hilar lymph node is not an indication of spreading cancer?

My opinion is as per the scan reports and inferences by radiologists, who saw it.
The hilar lymph nodes are most likely, due to your chronic smoking habit,which's a common finding.
