How To Diagnose Cancer At The Small Intestine?

default
Posted on Tue, 17 Jun 2014
Question: Good morning,
I have been recently diagnosed with a cancer at the small intestine. Surgeon removed 1 foot of small intestine where it was blocked. Lab test revealed type of cancer was an exotic one (rare type of carcinoma). During surgical intervention, 9 lymph nodes were removed and 5 out of 9 nodes showed presence of carcinoma. Since not all nodes were affected, it tells that cancer may be local meaning it did not spread out. Surgeon told me if someone has to get cancer, she told me that would be the one to get. It seems to be a TPN type. I asked bluntly what was the prognosis and she said without hesitation more than 80% probability of survival. I am 55 and in good shape. I eat well and do physical activities on a regular basis. I will go through a colonoscopy and gastroscopy to see if there are some carcinoma cells. What do you think? Am I in a good position considering my current situation. I do not feel pain nor fatigue. Thanks

Brief Answer:
Intesstinal Carcinoma

Detailed Answer:
Dear XXXX
It is very important to know what is the exact histology i.e that is type type of cancer. Did it arise from the intestinal mucle , fat or blood vessel of the intestine on which depends further treatment. Since there were lymph nodes also positive though they have been removed you still need some adjuvant treatment which will be on the basis of type of cancer.
Since the tumor was from exterior side colonoscopy will be futile and no malignant cells will be picked up.
Also there are some tumor markers which must be known for devising the treatment. I would personally recommend a PET scan to find if any residual or metastatic disease is present.
Since you are symptom-less and well at present I wish and pray that you continue with the same gesture.

Dr. J. Ticku,
thanks for the reply,
If i tell you that cell type is carcinoide, i guess it is too vague right? Also grade of cell is Grade 1-well differenciated.
For your information, i will have a blood test tomorrow and on the form it says Chromogranine A
please can someone answer back my reply , cell are carcinoid type or endocrinian type, Grade 1. what is the pronostic? What additional info do you need?

Brief Answer:
Carcinoid prognosis

Detailed Answer:
Dear XXXX
Thanks for reverting and informing me the histological nature of your cancer. Carcinoid as such doesn't arise from the external surface of the intestine. By trusting the biopsy report, carcinoid grade 1 needs only surgical resection. You don't need any other treatment at the moment except wait and watch. If sometimes there are endocrine effects symptomatic management is needed, which at present you have none.

Don't be worried.

Truly,
Dr. J. Ticku
