Question: Hi .. i would like to ask a question ..

Am 31 weeks and 6 days pregnant according to the doctor calculations .. but it shows at the ultrasound that am 29 weeks .. 20 days different .. the doctor told me that the water around the baby had increased to 30cm .. and the baby is small she weigts 1 kilo and 300 grams .. also they did ultrasound twice and they couldnt see the stomach .. the dr told me that they might need to deliever the baby earlier ..
Please let me know whats going on .. I'm worried .. shall i have to deliever ealier?? Is there anything wrong with my baby girl???

Hello dear,I understand your concern.In my opinion fluid about 30 cm means polyhydromnios.

For calculating the gestational age of fetus four parameters are considered like:-
1)Biparietal diameter(BPD).
2)Head circumference(HC).
3)Abdominal circumference(AC).
4)Femur length(FL).

If stomach is not visible in ultrasound AC will be less resulting in the less gestational weeks.

So it might be the reason for discrepancy.

Polyhydromnios is a condition where the fluid around the baby increases.Causes for it are:-
1).esophageal atresia- here the esophagus is not formed and there by fetus is not able to swallow fluid.
2).duodenal atresia.
3).spinal anamolies.
4)Diabetes in pregnancy.
5).Multiple pregnancy like twins.

Iam not diagnosing any condition in you but
just giving you the information.

Please forward me the ultrasound report for better assessment.

Kindly provide me the information I am ready for further discussion.

IUGR needs monitoring

Detailed Answer:
Hello dear,I got your information.I have seen your reports.

From the reports it is evident that there is symmetrical growth retardation where in the weight of fetus is less than average percentile.

This intrauterine growth retardation(IUGR) is monitored with:-
1)Serial ultrasound every 2 to 4 weeks.
2)Fetal Doppler.

If any severe IUGR is seen delivery is to be done immediately.

If mild or moderate IUGR is noted pregnancy is continued with monitoring.

But pregnancy is not continued out of the date.

You need to:-
1.Take bed rest in left lateral position .This increases the blood supply to fetus.
2.High protein intake like egg white twice daily.
3.Aspirin also might be helpful.

So dont worry with appropriate treatment and extra care after delivery results might be good.

Coming to esophageal atresia it might be diagnosed at birth.

And with prompt surgical correction prognosis might be good.

Am sorry but i have one more question .. am 32 weeks and 4 days .. but my baby measures 20 days smaller.. but the doctor told me that she's growing and gained weight .. doctor told me that she gonna keep the baby at least until 34 weeks and then she will see if its necessary to make me deliver earlier or to keep the baby inside ..

My question is .. if the baby born on 34 weeks ... would it be as same as babies that born on 34 weeks or 20 days smaller ??

Hello dear,I understand your concern.

In my opinion as per your reports the fetus has symmetrical growth retardation.
It means growth in all systems is retarted symmetrically.

So if the baby is delivered at 34 weeks it means the baby is actually 34 weeks but due to its retarded growth and less weight it is seeming less days than actual.

34 weeks is taken as margin because lung maturation occurs by 34 weeks.And if for any reason the delivery occurs at this point the neonatal complications will be less.

As your doctor said that the growth is picked its a good sign.If this continues and there is no severe IUGR pregnancy is continued even past 34 weeks.

Dont worry dear with good neonatal care after delivery the results will be good.

The doctor saw a small spot in the stmach area .. but the stomach not seen.. only a small black spot ..

That this means that the stomach is there but its not filled ?

Hello dear,I understand your concern.

In my opinion there might be some problem in swallowing or esophageal atresia due to which the stomach might not be able to fill properly and seen as small black spot.

Esophageal atresia is a condition where the patency of esophagus is not formed resulting in no connection between stomach and esophagus.

My aim is not to make you tense, But Polyhydramnios with small stomach might go in favour of esophageal atresia or other anomaly.

I am not diagnosing the condition ,I am just giving you the information.
But don't worry esophageal atresia is surgically correctable and have good outcome.

Everything is not diagnosed by ultrasound madam many things will be best diagnosed after birth .so I suggest you not to take any stress from now on.
