How Should Severe Stomach Cramps, Fatigue, Nausea, Black Feces And Dizziness Be Treated While On Several Medication For Acute Idiopathic Intracranial Hypertension?

Question: Hello, I have been taking a combination of 20 tablets as I was recently hospitalised due to some acute idiopathic intracranial hypertension. While in hospital for a week, they did a test of my stool and it was positive in H pylori, as a result, I was prescribed a lot of medication. Unfortunately, I've had to stop the medication today as it's making me feel incredibly awful. I'm experiencing severe stomach cramping, fatigue, nausea, black faeces, dizziness, etc. I can't eat nor drink anything and I'm finding it haee to even consume fruits, dry foods and water. Therefore, I would like a second opinion about the combination of drugs that I've been prescribed and what do you recommend that I do regarding my symptoms, etc.? Many thanks, Regards, XXXXXXX Please see the attachment for the drugs and daily dosage

Brief Answer:
Drug induced gastritis.

Detailed Answer:

Hello,

The symptoms you are describing are mostly suggestive of severe gastritis as a result of taking a number of medicines at a time.
You were on 20 types of medicines plus a combination of two antibiotics in very high dose with a proton pump inhibitor is added further.Usually high dose combination of Amoxicillin clarithromycin and metronidazole is given to treat H.pylori.But right now with 20 other medicines i don't think its a good idea to treat H.pylori.Its difficult for mucosa to tolerate such high doses of antibiotics with other medicines. Therefore my suggestion is stop therapy for H.pylori for the time being.
Once your medicines for intracranial hypertension are tappered off start taking H.pylori medication.

As of now add a ppi like Omeprazole 40 mg twice daily with antacid to provide relief from symptoms of gastritis.
Drink chilled milk to neutralise the acid in stomach.

I can't find any attachement so you have to repost them with follow up.
However a cocktail of so many medicines given at one time is not a good idea.

waiting for follow up.
Thanks.

Hello, I've had idiopathic intracranial hypertension for 13 years and subsequently 6 lumbar peritoneal and over 60 lumbar punctures in the past. However, in 2011 my shunt was ligated because I was also diagnosed with Tonsillar herniation as a result of the shunts. Therefore, it hasn't been working fot all these years. Unfortunately for the last 3 weeks I began to suffer from similar symptoms as before with visual disturbances and a headache that I could no longer control at home with the pain relief that I had. So I went to the hospital and consequently was admitted. While there, they did a lumbar puncture which revealed that my pressure was quite high... I haven't been hospitalised for my illness for over 5 years now; I've been pretty stable. I was prescribed with topiramate 25mg twice a day and Pantozol 40 mg twice for the idiopathic intracranial hypertension. I was also tested positive for H Pylori and as a result was prescribed: tetracycline 250 mg (2 tablets, 4 times a day), TriPotassium DiCitratobisumthate 120mg (1 tablet, 4 times a day) and Metronidazole 250mg (1 tablet, 4 times a day) so that all equals 20 tablets per day. I was also prescribed paracetamol and diclofenac which I take only when needed. However, feel absolutely terrible, excruciatingly stomach cramping, black faeces, nausea, dizziness, fatigue, loss of appetite, high temperature, etc. So I would like to know what I should do regarding the medication and what is possibly happening to me physically as a result? Many thanks Many thanks

Brief Answer:
Treatment is not good.

Detailed Answer:
Hello again my dear patient,
For treating H pylori the combination you have mentioned is a very old one used in 80's and 90's to treat it
tetracycline 250 mg (2 tablets, 4 times a day), TriPotassium DiCitratobisumthate 120mg (1 tablet, 4 times a day) and Metronidazole 250mg (1 tablet, 4 times a day).

Please consult another doctor and get a prescription for
Amoxicillin 1000 mg twice daily
Clarithromycin 500mg twice daily
Lansoprazole 30 mg twice daily
This combination is to be used for 21 days.
It consists of latest medicines which are very well tolerated by patients and eradication rate is also very high.
Stop the medications you are on.Your symptoms are side effect of medicines you are on.
Regards.

Thank you very much, doctor. Now I understand why I am taking a ridiculous amount of tablets as I recall receiving treatment for H Pylori a few years ago and I didn't take as many tablets nor feel as poorly as I do currently.

I just wanted to mention that I am allergic to penicillin; therefore, what would you recommend in place of Amoxicillin?

Many thanks once again.
