Question: I have a red blotch on middle toe, with welts on that toe and the one next to it. The toes are reddish and spreading to the foot at the base of the toes. I have a tingling feeling that is getting more painful and spreading to my foot and ankle. I initially experienced a sharp pain episode last Sunday while playing barefoot in the yard, but no noticeable cut, bleeding, etc. I have been applying the ointment each morning since Wednesday. It has been only mildly irritated until today. Now it's getting worse. I've just contacted my PCP via patient portal and await a reply. Also, there are pink spots on my foot up to 2 inches from the base of the toes.
I just uploaded a picture of my foot. I'm also feeling some tingling pain in my right calf.

Detailed Answer:

Hello,

It seems like something called starting stage of cellulitis. It is nothing but a spreading infection in the subcutaneous ( tissues underneath your skin). The small red bleh initiated the infection and spread to other areas later.
No need to worry as it will respond well with timely antibiotic treatment. Make sure that you have your blood sugar well controlled because uncontrolled diabetes can worsen the disease.
As of now you can start on antibiotics like amoxicillin / clavulanate combination for five to seven days. You can also take paracetamol in case of fever or pain. Apply topical antibiotic like mupirocin after cleaning with soap and water.
Generally the problem will be settled in few days, if it persist despite all these consult a physician and get evaluated.

Thank you, doctor. In the meantime, I talked with my brother, also a GP. He gave me essentially the same advice. I have some clindamycin on hand from an earlier issue, which I will start using 3x per day for a few days (took 1 already). I took a couple ibuprofen for immediate pain reduction, which is helping. Will follow up with acetominaphen as needed. Will stop using the triamcinolone and ask my PCP for topical antibiotic on Monday.

Continue clindamycin. Most often will improve with clinda, if not switch to other antibiotics.
