Brief Answer:
Rifampicin to be taken empty stomach.

Detailed Answer:
Welcome To HCM. I am Dr Anshul Varshney.
I understand your concern.
See, the kit you have contains 4 tablets.
One of them must be a red color capsule labelled as Rifampicin.
This one should be taken empty stomach in the morning and remaining others can be taken after breakfast.
Along with this you also need to take a supplement Benadon (Pyridoxine) to prevent peripheral neuropathy which is a side effect of Isoniazid, one of the component of AKT-4.
Another issue is Gastritis, that most people experience following consumption of these medicines. So we do prescribe a combination of Pantoprazole and Domperidone along to prevent it.
Check out with your doctor, he must have prescribed you that. In case not, discuss with him my opinion.
If you have any further query please ask me.
Stay Healthy.

Brief Answer:
Don't worry.

Detailed Answer:
Its simply Tuberculosis of Epididymis.
Many times I say, if you have to choose a Disease choose TB!! At least its completely treatable. Its not like diabetes, BP for which you have to take medicines life long.
It has no relation with alcohol , smoking and all.
Treatment duration depends on the severity. May vary from 6-9 months.
Yes, you can transmit it to your wife. So, I would advise you to get married after around 6 months.
Don't worry. You will be fine.
Medicines given to you are fine.
Just you need to add Pantoprazole for acidity. So, if you develop that start taking that too.
If you have any further query please ask me.
Stay Healthy.

Detail Follows.

Detailed Answer:
Dear Friend.
I am sorry ,I ha e no idea what this Tablet actually contains. Couldn't composition on internet also.
Just check on the strip and tell me what it contains. It seems to me a pain killer and anti inflammatory.
My concern is that has you mass been tested? I mean has FNAC or biopsy done?
If yes, attach the report. The report you are mentioning is of ultrasound. It doesn't prove that this is Tuberculosis.
If not be proved by biopsy or FNAC, a reconsideration of diagnosis is required.

Brief Answer:
Seems to Be Tuberculosis.

Detailed Answer:
Dear Friend.
I have checked your reports.
Reports are suggestive of Tuberculosis infection.
I would advise you to start on ATT. From my side your will be taking AKT 4 for 2months and then it will be reduced to only Rifampicin and Isoniazid for further 6-9 months depending on the response.
You can do night shifts. You can live normal life.
Start exercising regularly so that your body and health remains good.
You just have to avoid oily and fatty food.
Avoid spicy food.
Eat more of fruits and green leafy vegetables.
There are no other particular food restrictions.
If you have any further query please ask me.

Detailed Answer:
Dear Friend.
This tablet would help you in reducing the inflammation and pain also in case you have.
This treatment is fine.
You should trust your doctor, I feel that is the best way out to check the treatment is fine or not.
In case you have doubt you can always take a second opinion.
You can repeat your Ultrasound after 2 months and see if the size is reducing or not.
Based on available details, I see the treatment right.
If you have any further query please ask me.
