How Safe Is Paracetamol For Fever And Cold In A Child?

Question: My son is having fever and having cold since yesterday and i have given him CROCIN suspention 128 mg - 15ml thrice but still fever is there after every 3-4 hours, could you please provide the correct line of treatment urgently with steps and precautions.

CAN PARACETAMOL IS ADVISABLE OVER CROCIN PLEASE SUGGEST WITH MEDICINE NAMES.

Most common cause of fever with cough and cold in this age is some viral infection.Here fever remains of high grade for initial 3-4 days.
In most of the cases fever can be managed well with paracetamol. If fever does not respond to paracetamol even when given in appropriate dosage and frequency another medication like mefenemic acid may need to be added.
Crocin contains paracetamol(125 mg/5 ml) and the dose is 60 mg/kg/day and can be given at an interval of 4-8 hours depending on the frequency of fever.
The average weight of 12 years old child is about 40 kg.So each dose should be of 500 mg when given 6 hourly.
Currently you are giving 15 ml that is equal to 375 ml.I did not find the weight of your child but assuming it to be 40 kg the dose needed is more than what you are giving now.
So please adjust the dose according to weight and better consult a pediatrician for proper prescription.
Do sponging with lukewarm water as it helps in bringing down the temperature.
Your another query is that whether you may give paracetamol along with Crocin. As Crocin itself contains paracetamol there is no need of giving adding paracetamol.Better is to give mefenemic acid if fever does not respond to proper dosing of paracetamol and sponging.

Mefenemic acid is a prescription medicine so I request you to get a prescription from physician if it is needed.

Weight is 31kgs and giving crocin suspention aroound 15ml is ok or not please confirm and now he is not feeling hungry as well, what all can be given in food, please help with the correct medicine name as there is no fever for the last 4-6hours after giving crocin suspention 15ml.

But donot know if it gets repeated in evening so how long are we suppose to give medicine even if fever subside. please help with all details & precautions urgently.

As the weight is 31 kg,you may give 15 ml of crocin at an interval of 6 hours.
Decreased appetite is very common complaint during any febrile illness.
Once the fever subsides appetite increases.
Meanwhile encourage the child for easily digestible less spicy/less oily food.
Try to give fluids in sufficient amount to avoid dehydration.

Thanks very much for the reply, fever is reoccurring in 6hours now earlier it was 4hours and also CROCIN 128mg suspention is over we have already given him 60ml now can you please let me know the tablet that can be given to him along with the number of days for the treatment so that fever should not occur and he can continue to school from tommorow.

Hi

You may give paracetamol tablet 500 mg,3/4 tablet at an interval of 6 hours.Alternatively you may give double strength suspension of paracetamol like crocin ds which contains 250 mg/5 ml;7.5 ml at every 6 hours.
Fever usually starts decreasing within 24-48 hours.The frequency can be decreased accordingly with same dosing.As fever gets controlled paracetamol needs to be given only when required.

I have started given FEBREX suspention which contains Paracetamol 250mg, 5ml every 6hours but fever is now 102.5 after 6hours, can you please let me know in case of any change in medicine or line of treatment, roughly how long will more it take to subside completely.

Please reply urgently with details.

Hi

Fever may remain of high grade during intial 3 days in viral infection.The dose your are giving that is 5 ml(250 mg) is less.For 31 kg child the dose should be 7.5 ml(375 mg) 6 hourly as I have mentioned earlier.
If fever persists even with this dose mefenemic acid may be needed. Mefenemic acid is more effective medication for controlling the temperature.
Please consult a physician for prescription of mefenemic acid if fever is not controlled with paracetamol.Also if fever persists for more than 72 hour I would request you to revisit your physician to rule out bacterial infection or evaluation of cause of fever.
