How Long Should I Continue Medication For Thoracic Outlet Syndrome?

Question: I am being treated conservatively for thoracic outlet syndrome. I've been told to follow up to n 4 weeks if the rest and metaxolone, Celebrex and gabapentin don't relieve my symptoms. I haven't been able to sleep lying down for more than 2 hours for more than two weeks. A supine, prone or side lying position makes the pain and numbness worse. I get some reliev with an upright posture. How long should I continue with the conservative treatment approach. I don't think I can wait the requisite 4 weeks! The pain in my hand and shoulder is constant.

I read your question and i understand your concern. I believe you have neurogenic thoracic outlet syndrome and for that one should be kept on conservative treatment for at least 4 to 6 weeks of medications plus physiotherapy and see if it helps. Failure of response may then be considered for next management steps which are invasive and require surgery.

The next surgery step will be intended for decompression of thoracic outlet either by transaxial , supraclavicular or infraclavicular ( above or below clavicle / collarbone) approach. So waiting and seeing till 7th of july is advised.

Meanwhile focus on exercises provided to you by the trainer and comply to the medications and do following home remedies for the relief.

Maintain good posture
Take frequent breaks at work to move and stretch
Maintain a healthy weight
Try applying heat or massaging the painful area
Avoid carrying heavy bags over your shoulder
Avoid activities that worsen symptoms, or find ways to adapt activities so they don't cause symptoms
Create a work area that allows you to keep good posture and doesn't make symptoms worse
