Question: hey i would like to know if i can get any possible sexually transmitted disease or infection through the activity i did four days ago with a sex worker..i describe the scenario below:
1.I rubbed my penis(no condom) on her buttocks for around four minutes.i never got anywhere near the buttcrack or the vagina or the butthole.
2.i also proceeded to lick her whole body and
3.after that she put her pants on and i did the same.With our pants on..she started jumping on my penis and rubbed her vagina(wet) on my penis(with precum) (all with my pants on) for some three to four minutes.
i want to know the risk for
1. HIV
2. Hepatitis C
3. Herpes
4. HPV
5. Syphilis
6. Hepatitis B
7. Hepatitis A
please let me know!

I have gone through your query with complete diligence and analyzed the complete situation with best of my clinical acumen as well as the academic knowledge let me first and foremost reassure you that you have negligible risk of all sexually transmitted infections enumerated including HIV.

So just relax first and don't be so much anxious. Let me give you the scientific and medical explanations to my firm declaration of your healthy and clear state:

- In your case it was dry rubbing and not anywhere around anal or vaginal mucosa. Additionally at the time of jumping by female with wet vagina you have had your pants on so the complete act was done with clothes on. There was no direct touch of vagina to penis or any other mucous surface (like penis to vagina or anus) in the complete incidence. It is the most important barrier between the penis and vagina. In this world there has been no case reported so far with transmission of any sexually transmitted infections with clothes on.

- Now for the part of licking the whole body: There is no chance of any STD including HIV in this case. But academically saying we can say that there are chances of Hepatitis B infection. But clinically speaking and as per my experience I have never seen any infection after such kind of exposure. I do counsel your kind of cases where the person goes for repeated testing but I never saw any positivity for any infection including hepatitis B in any case.

- In the transmission of any type of sexually transmitted infection it is must that there should be exchange of fluid to fluid like blood or serum or vaginal or seminal fluid. As you can get there had not been any such type of exchange in your case.

During licking too our enzymes in saliva destroys most of the micro-organism including HIV if present.

The best part in your case was that it was only on the skin and not near any mucous surface. So do not worry and just relax.

In most of sexually transmitted infections it is the female who is at more risk than the male partner. So in your case you are not at the risk by your own secretions.

Having precum over the penis doesn't change the picture and again require some fluid or serum or discharge or blood from your female partner. Over the buttocks there are no such kind of fluids been present so is a simple situation.

Regarding unprotected sex with your girl friend: For the part of all the STDs except for hepatitis B there is no issues. But on academic grounds there is a negligible risk of Hepatitis B only. If you are immunized for Hepatitis B then the issue of hepatitis is also out of context.

But if you are not immunized then you should avoid unprotected sex with your girlfriend for a period of 6 weeks. Though I know that your tests would be negative but it is always better to be cautious. There is no need to go for repeated testing every week and a cut off limit of around 50 days is fine for the same to relive all the anxiety.

Now regarding the possibility of scratches using nails on arms and body: Again there are no chances of any of these except for a simple secondary infection. A nail injury can cause a bacterial infection later and sometimes hepatitis B.

Nails are basically keratin protein and the area from where one scratches are dead and lifeless tissues. So if you had not been immunized for hepatitis B then I would have given a risk for the same and not for other things.

As I know that you are immunised for hepatitis B so there is no chance of the same and also not for any other condition including HIV too.

Be careful for such kind of incidences and be cautious with the shady individuals.

In the above hypothetical situation also there is no chance of getting any of these infections.

A valid scientific reason for the same is due to the fact that every infection requires a minimal possible amount of viruses or bacteria which is called as "Infective dose". It is not possible by bedding or linens or corner of beds to attain this infective dose. So if this infective dose is not attained then the infection is practically not possible.

We so often visit so many places of hotels and restaurants or clubs. We never know who touched it are what all micro-organisms are present on those places. It is not practically possible to get any of STDs by such kind of exposure. Just forget the incidence and get on with your normal life and be careful with safer sex practice.

Regarding your further queries:

- In case of the condition if we consider that if food has someone else's saliva and blood and you have cuts in your mouth and hands: Again there is no possibility of any of these infections.

You must be wondering why !!! Blood or saliva contains direct microorganism then how can it be possible to be no infection. It is because of the medical fats that none of the microorganism can sustain the heat or the processing of the food. During the cooking of food all the microorganism causing STDs are destroyed.

Even if we consider that we eat a non-processed food then the mouth contains so many of enzymes which destroy these microorganisms including HIV too. When it goes in the stomach then these are further destroyed by acid in the stomach.

Now for touching the food with cuts in the fingers using the toilet and the toilet door knob has blood, germs, saliva on it : The first thing is that most of the micro-organisms get destroyed as soon as these come in contact with air. Also these cannot survive on these surfaces and becomes dead.

Last to last the most important of the good thing is the term of "Infective dose". Every of microorganism require a minimum of the infective amount which is actually not possible by any of these means.

I quite often guide my anxious patients that it is practically not possible to get any of these infections by the modes you have asked above. If such a mode could be possible then more than half of the world would had been suffering from one or the other sexually transmitted infections......which is actually not!!!

I must say that you are lucky person or you might be thinking that each and every time you are putting the situation....it comes out to be clear !!!

So in the present scenario given above too there are no chances of either syphilis or herpes. As I can get that you have had dry kissing that too for very shot period of time of three to four seconds. In this type of kissing there is no possibility of transmission of herpes or syphilis.

The type of kissing you mentioned is a dry kissing where there is no exchange of fluids. It is same as a mother kisses a baby or gentle affectionate kissing which we all do as good gesture. Had it been a case of wet kissing and deep kissing there had been definitely chances of acquiring one or the other problem due to exchange of saliva.

I don't thing that the lesion on the lip has any relationship with any type of sore of herpes or syphilis. But even then for the best confirmation I request you to send few good resolution pictures of the lip area form close up view. Make sure that the pictures are good resolution(and not blurred one) which can be magnified for better analysis.

the cut/thing(i dont know how to describe it) is too small to be captured on the camera..it does not hurt and i cannot even feel it...

The cut kind of thing doesn't appear as herpes at all. In herpes there are fluid filled vesicles are present which are quite painful. So you can judge that the condition is not at all related to herpes.

In the context of all the above situations also there are no chances of any of STDs. In all these conditions the microorganisms if present gets diluted and becomes avirulent so it vanishes the impact of any microorganisms. The situation you have described does not transmit HIV or any other STDs. Therefore you can allay fears about transmission of all STDs.

As I said previously the only chance if had been there was for Hepatitis B only but as you are immunized so makes you completely free from all STDs.

Let me tell you certain important facts related to herpes:

To tell you medically - Herpes lesions manifest itself as small vesicles (fluid filled blisters) which remains for just a day or two and then forms ulcers which are quite painful. These ulcers then heals in next 5-7 days. The common sites for herpes are either near lip r or genitals. It never manifest on back.

To begin with also these vesicles are quite painful and itchy with discomfort and once the vesicles burst in a day or two then the ulcers which are formed are quite painful for next 4-5 days. So the symptoms can never go unnoticed.

In yourself I can make out that no such symptom is present matching with the above symptoms.

i reviewed all my risk incidents and recalled every incident except one.

I always roam around shirtless in my room.i always roam around without a shirt.Due to playing contact sports ,i have lots of cuts on my chest .
also due to bad habit of chewing my lips ,i always have cuts on my lips.

i have a microphone attached to my computer which more than dozen people use..some people known some unknown.i use the microphone daily and i have the habit of rubbing the microphone on my lips while using them and i also rub the microphone on my chest if i m talking to someone before sleeping.
my question is what if someone got their blood,saliva or sweat on the microphone and i used the microphone right afterwords and started rubbing it on my chest and lips(both of which have cuts).what if the saliva and blood and even sweat somehow get into my cuts on the lips or chest .

In the context of all the above situations also there are no chances of any of STDs. In all these conditions the microorganisms if present on any non living things becomes dead and therefore the impact of any microorganisms is vanished off. The situation you have described does not transmit HIV or any other STDs. Therefore you can allay fears about transmission of all STDs.

As I said previously the only chance if had been there was for Hepatitis B only but as you are immunized so makes you completely free from all STDs.

i have a few questions.

recently i visited a brothrel.I did not have sex with anyone .But a few things happened that concern me.

1. I scratched my leg against a bed.It was rough.The sides of that bed were rough and sharp and they scratched my leg. i inspected my leg soon after,i couldnt see bleeding but it definitely punctured my skin and removed some skin. what if some prostitute or some individual with stds also cut themselves at the very place on the bed which cut me...and it has their BLOOD,SALIVA AND BODY FLUIDS...and when i cut myself the blood,saliva and body fluids got into me and transmitted stds into me.



2. Secondly in a bizarre situation..while i was talking to this prostitute...she spit on a pillow and moments later proceeded to hit me with the pillow in a playful way.While doing so , the wet part of the pillow rubbed against my lips.The wet part of the pillow(due to her saliva) definitely rubbed against my lips. And my lips have many cuts on them due to habit of chewing my lips. what if the saliva or blood from her mouth( infected with std's) which wet the pillow and was rubbed against my lips which has cuts transmitted some some sexually transmitted disease...what are the chances of me being infected with any stds in the incident?



3.I also massaged her legs using oil. i also have the habit of chewing the skin on my fingers and i have some cuts on both the fingers.what if she had some vaginal fluids or even menstrual blood on her leg or the saliva of a guy she had sex with before i met her and i massaged her and the infected saliva,menstrual blood,vaginal fluids got into the cuts on my hands and infected me with sexually transmitted diseases?

4.She also scratched my legs with her nails and this time it did bleed( in three places on my leg)..i could see clearly. this is extremely risky i guess. what if she was fingering herself before i met her and she must have had some guys sperms on her hand or even her own blood.

The question is " she did not clean her hands and scratched my legs with her nails and i bled in three places..what are the chance of transmitting std's in this way"?



5. Also after bieng scratched by her nails and bleeding at three places...my leg rubbed against the underside of a table at the same spot of the three cuts quite roughly aggravating the cuts.

I have mentioned before that i have cuts on my hands because of the habit of chewing on them.when i entered the room...i removed the bedhseet with my hands and the bedsheet definitely rubbed against my fingers which has cuts...lots of people have had sex on that bed and their sexual fluids would all be on the bed...what if the sexual fluids,sperm,vaginal fluids,menstrual blood,Saliva of someone infected with any std (hiv , hepatitis C) got into the cuts of my hand and infected me with the diseases.

Also i would like to add two more incidents which happened...

the Prostitute pressed my body quite roughly against the wooden bed support/the body of the bed...she pressed my body against it so hard that the bed support made an impression on my back(if you understand what i m trying to say)...the bed is used by thousands of customers... the bed had some infected person's and the prostitute's blood,sperm,vaginal fluids,saliva sweat and it somehow got into my back(i have back acne and scratches too)
and infected me with sexually transmitted diseases.

Also the second incident is I USED THE XXXXXXX PHONE OF THE PROSTITUTE...and as you know i have cuts on my fingers due to bad habit of chewing on my fingers..what if the XXXXXXX screen and the XXXXXXX phone had saliva,vaginal secretion,blood, sperm and sweat on it of some infected person and the prostitute's and somehow the virus and the bacteria of the infection got into the cuts i have on my fingers and infected me. what are the risks..

I completely understand your deep concern for each and every situation.

I analyzed each and every of situation provided by you and after proper analysis of each situation individually I can make out that none of the situation is actually a risk factor.

On the basis of my medical knowledge as well as my clinical experience of so many years, I reassure you that none of the above five situations as well as the latest one asked just now is not going to prove any serious issue in your life.

As I already guided you that in most of your situation either there are inanimate nonviable surfaces are present or in rest of the situation there are the important matter of infective dose of the microorganisms.

So as I briefed you before that these nonviable surfaces are not prone to cause or transmit the infection in all the modes which you have asked. These all are basically trivial and normal modes of contact which we all have in our day to day life.

I guided you that in our clinical practice too which touch and examine so many patients and actually we don’t know the actual status of the patients. So in that scenario we should have been at maximum risk....which is actually not.

Including this too we see that during our student life in college or school we stay in hostels and dormitories where there is lot of contact with one thing or other things. If infection of this type could have been possible then most of the young people and students had been suffering from one or the other STDs.....which also is not the actual scene!!!

So after analyzing these entire situations, I reassure you that at this time there is no possibility of any infections. But I also want to give you a brotherly suggestion to improvise your daily routine as well as cut down your risky behavior.

Dur to scratching deeply by nail there are chances of Hepatitis B. But in your case as the lady scratched you such hard that it caused bleeding then also the chance of any of STDs are not present. As I said earlier this requires minimum amount of infective microorganism to cause the infection and in scratches the only possibility is of hepatitis B. But because of your immunization status the chances are not present.

In the rest of the situations too there are no chances of any of STDs including hepatitis B also as are our day to day routines which are just part of normal hygiene. So there are no chances of any of STDs in you case.
