Posted on Fri, 10 Apr 2015
Question: My father is 75 years old and underwent CABG 4 months back. For his sugar control has been prescribed linagliptin 5 mg alongwith mixtard 30/70 insulin and metformin.
His sugars have improved but he suffered a UTI a few weeks back. Could it be because of Lingliptin and is it safe to have linagliptin for long term

Brief Answer:
No it is not due to gliptins

Detailed Answer:
Dear friend, greetings from HCM, I understand your query and have gone through the reports you attached. Diabetics that too under stress do suffer from infection as diabetic patients will have less immunity.
You give him a course of Tab. Nitrofurantoin 100 mg two times after food for five days.
It is also very important to have a very good personal hygiene so as to reduce chances of any infection. Also they are prescription drugs, so please get a medical prescription for that.
Thanks and do contact me in case of any more queries.

His infection is better now. So that means Trajenta is ok to be had in long term.

Based on his reports does he appear to be a case of diabetic nephropathy..his creatinine levels have been in range always.

Brief Answer:
Yes, then no need for antibiotics

Detailed Answer:
Dear friend, greetings from HCM.
Yes his Renal profile is now OK. But check his microalbuminuria which is the best indicator of diabetic nephropathy. If he has significantly high values he needs angiotensin receptor blockers or ACE inhibitors to be added .
since he is in stress now glucose levels will vary. You need to keep him under regular follow up.
You please monitor his sugar levels regularly for one month. Record his fasting and post meal glucose levels and the dose of insulin given too. and later meet your consultant and get the proper dose fixed. He needs proper lifestyle modification too.
Thank you
