Posted on Sat, 5 Apr 2014
Question: Dear dr, My wife is likely 2 or 3 weeks pregnant (from the date missing period) pregnancy test kit positive. We went to a GP and had an ultrasound. A 5cm ovarian cyst was found and the dr referred is to a Gynae . The baby is too small to be seen according to the dr. We are really terrified now. My wife have an ultrasound 5 months ago when some constant brown fluid remained for many days after get period was supposed to end. Ultrasound was clear then .. How can the cyst grow that fast ? How common is ovarian cyst during first stage of oregnancy? What complications will it have ? Will the baby be affected n in wat way in early stages? Or late stage? Can the cyst be removed now? If yes, will a miscarriage happen ?? If the baby grows n the cyst get bigger , what shld be done? Pls tell me what to prepare . We are waiting for a gynaelogist appt n would like to know what to expect first. Thanks in advance

Brief Answer: DETAILS ARE GIVEN BELOW. Detailed Answer: Hello, Thanks for writing to us. Followings are my comments: 1) First of all, your wife is more than 6 weeks pregnant from last date of period (as per given information). Ovarian cyst is seen in some percentage of pregnant women. Most of them are benign in nature. Ovarian cyst usually grows slowly over the time. If previous ultrasound scan (5 months ago) was clear & normal, then presence of 5 cm cyst or rapid growth should be properly evaluated through Color Doppler Trans Vaginal Ultrasound scan. It will helps to determine blood flow and nature of the cyst. Increased blood circulation during pregnancy may be the cause of rapid growth of the cyst. Kindly, consult with Gynecologist for above said investigation. 2) An ovarian cyst normally develops in early pregnancy to help support the pregnancy until the placenta forms. In some cases, the cyst stays on the ovary until later in the pregnancy. Most of them are benign in nature. 3) Nature of the cyst should be evaluated first. Benign cyst doesn't produce as such complication during pregnancy until it is hugely enlarged (more than 10 cm). Torsion, rupture of the cyst, infection, intracystic hemorrhage etc are common complications. Periodic ultrasound scan should be performed. 4) NO, ovarian cyst DOESN'T hamper baby's growth and development either in early or late stage. So, you wouldn't be worried about that. 5) It is better to monitor the ovarian cyst periodically by clinical examination and ultrasound scan. If cyst is enlarged more than 10 cm and produce clinical symptoms like dull aching or severe pain, heaviness etc, then cyst can be removed in the 2nd trimester (best) after consultation with her gynecologist. Otherwise, cyst can be removed during ELECTIVE C-SECTION at the time of delivery of the baby (most cases). 6) Periodic evaluation of the ovarian cyst is must as the pregnancy progresses. Decision should be taken according to the stage of pregnancy & status of cyst. MY SUGGESTION: I suggest to consult with GYNECOLOGIST regarding this and do periodic check up with above said investigation. Don't be panic or worried unnecessarily. She must start to take FOLIC ACID and VITAMIN supplement with protein rich diet, drink plenty of water, avoid heavy work/ long journey and take adequate bed rest. Hope, it helps for your information. Kindly, close the discussion if you don't have any other query. Wish her good health and successful motherhood. Thks for ur earlier replies. The dr did a check and we were told to kiv the cyst. The dr said that it could be hormonal cyst but usually less than 4cm. In the case of my wife is 5.7cm n unlikely to shrink. The cyst look clear black. What are we expecting in the near stages? Is eating prawn n squid safe? I need to drive a van at times and I'm afraid that the rocking on uneven road will affect the baby in such early stage? My wife is feeling fatigue most of the time .

Brief Answer: DETAILS ARE GIVEN BELOW. Detailed Answer: Hello, Thanks for additional queries. 1) Do periodic check up with ultrasound scan (USG) to monitor the cyst and overall growth and development of the baby. Most cysts are BENIGN in nature and usually DON'T grow rapidly which needs surgery in the 2nd trimester. 2) Certain fish contain high levels of mercury which can affect your baby's developing nervous system. I suggest to avoid RAW SEAFOOD, Prawn, Squid, Crab, Shellfish etc up to 16 weeks of gestational age. Well cooked fish can be eaten on regular basis. 3) Long journey with uneven road surface should be AVOIDED during the first trimester or up to first 15 weeks because it is the most VULNERABLE period of pregnancy. Chance of miscarriage is quite high at this stage. She must avoid heavy work and take adequate bed rest with protein rich diet. Hope, I have answered your query. Kindly, close the discussion. Good luck and all the best. Regards, Dr Soumen
