How Can Skin Issues On My Inner Thighs Be Treated?

Question: Hello Doctor,

I have noticed skin issues on my inner thighs (both side) seems to a fungal infection, but not sure. The skin color turned really black in that area. I just want to get rid of that infection and color to be like normal skin color. Can you please suggest me the solution ? I really feel uncomfortable while walking and I get lot of sweat between inner thighs. I want the infection to be reduced and the skin color to be normal as remaining skin area shown in the picture.

I have seen the photos. This seems to be lichen simplex chronicus or Acanthosis nigricans. Both conditions have different treatment. I need moe details to differentiate between the two.

Is there itching? Any other symptoms? How long its been there? What are constituents of KG cream?

Hi Doctor,

Is there itching?

Yes, at times the itching level goes to peak.

Any other symptoms?

Even though I dry the groin area, it gets sweaty very soon. I feel a lot of sweat while I walk. I feel a burning sensation too.

How long its been there?

It's been like 6 years, initially it was like the size of a INR old 25 paise coin. Now it has spread to some more extent.

What are constituents of KG cream?

Sorry Doctor it was KZ Cream not KG Cream...

Composition for KZ 2% Cream
Ketoconazole Topical (2% w/w)

Please help me get rid of it. I will be really grateful to you.

Please have a closer look at all possibilities and suggest me a good & permanent solution. I'm planning to get married soon, i don't want my partner to feel uncomfortable because of me.

This is most probably Lichen

Detailed Answer:

Hi,

Lichen and tinea can resemble each other. Both tend to be chronic (long duration) and may recur. Since in your case anti-fungal cream (KZ cream) didn't have an effect, its most probably Lichen.

Lichen gets worse with itching. I will suggest a moderately potent steroidal preparation for controlling the itching and the rash. You can apply Betnovate cream 2 times a day for 3-5 days.
If its showing good effect then continue till the rash is minimal for maximum 10 days. After that, apply only on and off when itching is there.

OK. I will do the same. What if the creams fails to show effect ? what should I do ?

Shall I say Betnovate Cream at Medical Center or is there any Chemical name for it ? If Yes, please let me know.

Hi

If the cream shows its full effect, then stronger antifungals for tinea may be needed. But I think Betnovate will suit you. Yes you can ask for Betnovate cream at pharmacy, its widely available.
If the rash goes fine, You can leave it alone. On and off is needed when you feel its (redness, itching) getting out of control.

No lifetime is not needed. When the rash diminishes, the skin stays normal for a long time, until you itch the area again. Thats why you need to avoid any friction at the area. If needed reduce weight.

Yes skin can be normal or almost normal again. Hygienic food, clean drinking water, goes a long way in keeping these problems away.

100g will be needed as its a bigger area to cover.
