Question: (History)

sir i am diagnosed with diabetes type 2 1 month back. it was shocking for me because i was not in any kind of problem or any major symptoms which i know about diabetes. In a random blood sugar test my reading were 230 mg/dl, and in fasting blood sugar war 250 , and hbA1c test was 9.7. it was seriously high and i was in shock. In my family history my father was diabetic in late 50s of his age and heart patient, but my age is 32 right now, and i am not over weight(63kg 5 feet 7in height). Also no obesity type problem.
Doctor give me medicines one 500 mg metformin daily at night and one Atrovastatin 10mg in morning( for cholesterol). After these medicines and a complete change in my diet i am now at a range of 100 to 160 mg/dl (pre and post meal) blood sugar level. Eliminating or taking very very less carbohydrates and mostly i am taking protiens and vegetables.

Question;
During my diagnosis , sugar was detected in urine . But now after one month of full control no sugar in my urine. But sir now new problem came. aectone found in my urine. doctor told me its due to starvation and due to less drinking of water. because as per my doctor there is no sugar in urine and my fasting sugar level was also 107 mg/dl , so she just give me one drip of normal saline iv type something. just to flush out . sir kindly tell me is that acetone is dangrous even i have controlled sugar level.. or it may be a reason of high protien low carb diet? or due to some skip meals? is that possible.?

Brief Answer:
Yes,it is possible

Detailed Answer:
Hi
I have gone through your query and here is my opinion.

If you follow a hypocaloric low carb diet,if you skip meals,if you feel hungry and you do not take food and water;the result will be normal-low blood sugar levels and ketones in urine.

In absence of carbs(during starvation or in low carb diets),your body burn stored fat instead of glucose. This metabolic process is called ketosis.

Ketosis can be dangerous (even if you have normal blood sugar levels)because it can produce metabolic acidosis or diabetic ketoacidosis. This condition can be treated only in hospital and it can be life threating.

In your case, you have only ketosis and the situation can turn normal if you take corbohidrates,fliud/elektrolite replacement and your insulin therapy.

Hope this is helpful.
If you have other doubts, feel free and ask.
Take care.

Ok dotor thanks for your reply. But now as i have told you i am recently diagnosed. And from last one month i am only taking one tablet of metformin 500mg. But no insuline. I dont understand why told me about insuline. And my blood sugar level is also controlled with diet changes.
And also kindly tell me what kind of diet i should take.? Bcz i shifted to low carb diet and now this acetone problem came. My mistake is i have not taken Breakfast from some weeks. Is that could be a reason?

Brief Answer:
50-60 % of your daily calories should come from carbs.

Detailed Answer:
Hi again
I have reviewed your attached report.

It is evident that your Diabetes is under control with 500 mg metformine and a strict diet.
Low carb diet and skiping breakfast has produced ketones in your body and the result is acetone positive in urine.
As i said, this problem autoresolve if you take food, water and insuline(or metformine in your case)

A diabetes diet simply means eating the healthiest foods in moderate amounts and sticking to regular mealtimes.(not skiping meals)

A diabetes diet is a healthy-eating plan that's naturally rich in nutrients and low in fat and calories.
Key elements are fruits, vegetables and whole grains.

In your circumstances,i suggest to talk with a dietologist for the most apropriate eating plan for you.
About 50 to 60 percent of your total daily calories should come from carbohydrate. Carbohydrate contains the most glucose and gives the quickest form of energy. So your body, will not use fat to produce energy and you will not have ketones in your urine any more.

Hope this is helpful.
Wish you all the best.

Ok doctor i understand. But if my ketones are not due to diabetes and only because of skiping meals and low carb diet. Does still it is dangrous? I was on some keto diet because i found it very easy to contorl my blood sugar.

Brief Answer:
Keto diet is the cause of ketons in urine test.

Detailed Answer:
Hello
The keto diets are helpful to controll blood sugar levels but at the other side these diets induce ketosis because they force body to burn fats rather than carbohydrates.

If you are in a keto diet ,i can say that your ketosis
is the result of the diet.
To resume: you have nothink to worry about because the ketosis will disappear soon if you eat three meals /day + two snacks with 45-50 gr carbohydrates per meal.

Hope i have clarified all your doubts.
Let me know if i can assist you further.
Regards

Brief Answer:
Yes,it is possible.

Detailed Answer:

It is true that one of the major factors affecting your blood sugar is the type and amount of food you consume.Other important factors are exercise, stress and illness.

The recommended "portion " of carbohydrates are 50–60 grams per meal and 10–20 grams per snack, totaling about 150–200 grams of carbs per day.

So don't worry, even if you will take 40 gram carb per meal, you will have normal blood sugar levels.

Hope this helps.

Doctor may i could not explain you..after 40 gram carb how could be my sugar level normal?
Last day my morning fasting sugar was 109 mg/dl, and i drink one cup of milk and after 90 min my blood sugar was 170. I think it was only 10 gram carb inside.
But if i take same amount of milk afternoon my blood sugar is not going more then 130.. What could be the reason?
And how i can tak as much carbs as u mentioned like 150 to 200 gram? That will be too much for me.

Brief Answer:
Recommendations.

Detailed Answer:
Hi again
Considering your weight,1 gram of glucose will raise your blood sugar about 5 mg/dl no matter what your blood sugar may be.

If you eat 2,000 calories a day, you should consume about 200-250 grams of complex carbohydrates per day.It is not too much.It is the right amount of carbs for the normal metabolic processes. If you eat less carbs you will have the riscs of ketosis because your body will use fat(instead of carbs)to produce energy.

According to the dietary recommandations of XXXXXXX Diabetes Association,a good starting place for people with diabetes is to have roughly 45 to 60 grams of carbs per meal and 15 to 30 grams for snacks.

So,Don't worry! You can eat 40 mg carb every meal and have normal blood sugar levels.
Apart the food,Physical activities and the level of stress(physical or phsycological) are key elements that explain why you have different blood sugar levels in the same moment of the day,in different days.

Hope i have answered to all your questions.
Wish you all the best.
