Hi There I Need Help As I Am Scared To

Question: Hi there
I need help as I am scared to death about my symptoms.
First some background. About 10 years ago I got terrible cough that lasted like 9 months, woke me from sleep, it was terrible. Pulmologist said my lungs are ok and ENT doctor told me I seem to have GERD problems and the acid was going to my throat and that is the problem of the cough. I got some proton pump tablets and gradully it went away. But under my back tonsils there stayed a whitish strip from tonsils to the throat, more on the left. Also my nerve that goes from my ear to my throat I suspected was damaged because of all the coughing for so long. Left side seemed somehow more problematic than the right, for example going with the cotton swab to clean my ear (I know that is not the thing to do) I had to cough always afterwards. Nervehere seemed more problematic than the right one.
I also have what they call broken nose and postnasal drip since that coughing. The doctors told me they can operate if it is not under control, but I somehow manage to clean my nose with saline and at the morning I have some clear mucus from my sinuses so I have to breathe it in and expel it through mouth. Some days it is worse, some days it is ok, I got used to living with it. When I get checked by the doctor she says my throat is somewhat red and it is like chronic by now, I always have a little red throat even without pain.
Fast forward to last week I think about 6 days ago. I am 51, male, nonsmoker, I did use to drink more than advised for some years but never to the drunk state, last half year I do not drink alcohol at all. I also have a history of health anxiety, mostly cancer related.
In january I got few virus infections of my throat, left side is usually worse that the right side since 10 years ago as I told at the beginning. During the coronavirus now in March I didn't have any infection of the throat, I would remember it because I was feared of the virus. Of course I didn't look at my throat I didn't have any problems.
Suddenly about 7 days ago when I swallowed food I got sharp throat pain but only on the left side. At the same time I got left ear ache, at first it was more stabbing and now it is more like ache. At first I said to myself, that is the "broken" nerve since 10 years. But as days passed by at some point I got scared because usually I don't have throat problem on one side only when eating. Well, somethimes I have a little pain on just swallowing, but it may be because I am overlistening again myself because I have so much caner fear now. But I remember the pain started at the moment, I mean like wednesday no pain whtsoever and on thursday I got sharp pain when eating. It was like an on off switch. I got terrified when I looked at my throat. It looks like my left tonsil has a part that is bigger kind of a lump, but not that big, I mean a little erected from surface and it looks like a little grey cloudy there. On the right of my left tonsil, so the throat itself it is quite reddish, the right side is much less reddish (as I told I have a little red always. The white strips from behind the tonsil down (barely visible if I do dont open mouth on specific way so the tonsil gets more visible) is still there, a little under the right tonsil but like 5x less. I know this was there for years and doctors never said it is anything wrong, but my tonsile enlargement and a little of cloudy color there I am not sure. My tonsils were like the doctor said some years ago quite "eaten up", like they are in a bad shape not functioning very well, because of the reflux they thought. Anyway, I googled reffered pain in the ear and one side pain with swallow and I got tonsils cancer out :-( As having a cancer health anxiety it is quote bad now, I am just listening to my swallow and ear and I am not sure if I am making my own earache now or it is really there, it is not as sharp as a week ago, but the sharper pain when swallowing is still on the left side. Near tonsil (from behind it to the center of the throat) there is still reddish and seems like it is somewhat swelled. The tonsil with enlarged "lump", sligtly on the grey is still there and it has maybe been a long time, as I had no pain when swallowing I didn't look at my tonsils so thoroughly, when I had a cold I just look at my throat if it is more red than usual and swollen or not. This time it is a little swollen on the affected side and when I swallow salive sometimes I get a burning fealing like regular sore throat, but only on left side... The one side is bothering me as I now that can mean cancer.
I have some neck pain but this is also quite frequent for me, this time it is back neck pain when turning head down, not left or right. I checked my neck and under the jaws extensively and I am quite certain I do not have any enlarged lymph node. On the other side of the neck I feel it a little on the problematic side I feel nothing at all, no lump or anything. I have no blood in the saliva. It is not hard to eat, it just feels sharp pain when I do and that scares me so much.
Can there be any other explanation of this symptoms except cancer of the tonsiles, throat, I don't know where, tonsile is susipicios to me and I am not sure if this redness of the throat can be cancer. When swallowing saliva it feels burning if it feels something.
Such symptoms I never had and I am really scared now, and the doctors work here only on coronavirus for now. If it is cancer, and they say once you have pain it is quite too late. Does the pain from cancer comes so fast, like one day ok, next day sharp pain when I eat?
I am cycling some time, didn't notice any other major problems, more mucus in the morning from nose because this corona made me very very stressed and I had some stomach burning pain and some chest pain (GERD). Blood was examined last time 2 months ago, general MD looked at my throat 3 months ago for some other problem (stomach flu) and didn't say anything about my tonsil. I have gained about 2 kg now when I am more inside that before.
Sorry for the long letter it seems ok to tell you my history and more details, as you can not see me in person. I tried to get picture but it is very very difficult to take it and something can be seen, it is not like the pictures of the stage IV cancer I've seen on the net that there is like a half throat lump or something, so I would need a good light for you to see on the camera. It is visible of course, it is just hard to take photo, if you need I'll try to take something as best as I can.
Thank you in advance for your answer
Yours Sincerely XXXXXXX

Brief Answer:
This is not cancer of the tonsil.

Detailed Answer:
Hi,

Thank you for your query.

1. The nerve supply of the ear is connected to the head, neck and upper half of the body. Hence it is normal to cough if you try to clean the ear. Similarly throat pain is referred to the ear and vice versa.

2. Many patients have been sending clear close-up images of their throats here on Healthcaremagic for more than a decade. It is best for someone else to click a focused picture using the flash. The images help in making a more accurate diagnosis. This is also because there is no standard description or appearance of the throat.

3. Cancer of the Tonsil is very very rare. It does not present like this.

4. The redness, white strips and the appearance of the tonsils is most likley to be granular pharyngitis which is a very common.

5. Hence you should upload clear images of your throat. It will help me give you an accurate diagnosis. Then further treatment options can be discussed.

I hope that I have answered your query. If you have any further questions, I will be available to answer them.

Thank you for your answer.

I am not sure even if the pain is from the tonsil, as the food passes somewhere it feels exactly like I would eat something that would scratch my throat, like old crust of bread. Which I did not and it wouldn't last a week I guess... That is like what pain feels like, but I am not sure where it is coming from, maybe tonsil, if the food passes it. Or back of the throat...

Is not feeling any enlarged lymph nodes or lump of the neck a good or bad sign?

I am alone in quarantene because in Slovenia we social distance now. I will take some kind of photo and upload it. Close up images look scary to me and my throat around tonsils lookes more and more problematic each year.

I hasve managed to send 3 photos where the tonsil can be seen.

Natural flash - I used the flash, there on the right side of the image (which is left tonsil) the the grey part that is standing out a little can be seen and also there is a small white patch further up the tonsil and under behind the tonsil the start of the white stipe that goes down to the throat can be seen (this stipe I have for years, doctor told me it is mucus, but it never goes away, it is there on the right side also, but it is 5x more prominent on the left).

More intense flash - made better sharpness of the image but lost some of the color I guess

Night mode - my photo has great night mode that takes more images and lightens them. Image is very sharp, but it has lost the white on the tonsil.

The first image is I guess the most real presentation of what I see. I think the redness and swelling to the right of the left tonsil (where the moving flesh is that stretches left and right) can be seen though, maybe on the more intense flash, as you can see the skin behind tonsil seems more red, but the camera didn't catch it fully unfortunately.

Sorry to write again I think my new image redness.jpg is the best to see "swelling" and redness behind left tonsil, the white below the tonsil is not seen because of the tongue and I have to put throat special position that the white below comes out. I have this white for years, I am not sure about tonsil.

If I wouldn't have sharp scratchy pain when I swallow food and some ear ache and read about this 2 symptoms are cancer, I would not bother by the looks so I am not sure if had different throat a year ago by the looks.

Thank you a lot for your time to read all my letters.

Brief Answer:
Details as described below:

Detailed Answer:
Hi,

Thank you for following up.

1. Your throat looks within normal limits.

2. The pillars of the tonsil (basically normal muscle folds) are slightly congested. The pale areas represent granular pharyngitis. Mucus is seen and stringy in the last image.

3. These changes are consistent with chronic tonsillitis, allergy and acid reflux and are very common.

4. I suggest a throat swab, PCR swab, allergy (IgE) and food intolerance (IgE) tests, tests for h.pylori and a USG abdomen.

5. Routine medication such as anti-allergy, anti-reflux medication, medicated gargles (oral rinse) along with allergy and diet control will settle this issue. Antibiotics only if the throat swabs are positive. Recovery may take many months but will last if you follow this protocol.

I hope that I have answered your query. If you have any further questions, I will be available to answer them.

Thank you for your follow up. So you think on what I described and have seen on the images, that this is not cancer (I know you can not be 100% positive of course without examination)?

The one sided pain, as you can see maybe left tonsile is at some point enlarged like some blister on it with greyish dim, more redness near it, and reffered ear pain got me worried in the first place.

I was tested for hb pylori 5 years ago and was negative. I don't have food intolerances, was tested because of my gi problems years ago.

What is a throat swab and PCR swab, is this for coronavirus, can this be some coronavirus presentation? What is USG abdomen? Ultrasound, I think I had this 2 years ago and everything was normal.

I am even not sure my throat hurts when eating at the tonsils, I feel like it is lower down , I am not sure, it has to do something with the ear throat nerve as I have some ear discomfort this week on the left (sometimes also on the right).

Thank you

Brief Answer:
This is definitely not cancer of the tonsils.

Detailed Answer:
Hi,

Thank you for following up.

1. This is definitely not cancer of the tonsils.

2. The pain while swallowing is typically due to mild active on chronic tonsillitis. These appearances are common. The blister like congestion will cause transient referred ear pain. It will settle down after treatment.

3. There are PCR swabs for many bacteria and viruses available, including common coronaviruses strains such as OC43, HKU1, 229E, NL63 which are not part of this pandemic.

4. Most of your other tests seem to have been done, so no further tests required.

5. Follow the treatment protocols mentioned in the previous answer. In addition if the patient keeps complaining about pains lower than the tonsil, I do a Videolaryngoscopy (VL-Scopy). You can share images of a VL-Scopy here.

I hope that I have answered your query. If you have any further questions, I will be available to answer them.

Thank you for your answer. So ear pain is because of tonsil problems and this does not seem like cancer on the image.

Cancer can of course still be below in the lower throat which can not be seen here? Because I am not sure where it hurst when swallowing, I do have some hoarse voice on and off for years, nothing much, I thought this was from my gerd/post nasal drip problems that I have. Sometimes I have sore throat and sometimes I have pain when swallowing lower at the end of the neck. But this is for a few days and stops and this has been going on for like 10 years (sometimes a little hoarse voice and lower neck pain when swallowing saliva and food).

I mean, this is probabaly more chance to be gerd/post nasal drip related than cancer as it lasts so long and on and off and I have a lot of clear mucus for years now from throat (in the morning the most).

But ear pain is not from further down the throat, the nerve is near tonsils, it is from my tonsils problems?

When my stomach hurts usually I have some chest pain so I do have GERD problems and now my stomach is doing worse because of my stress over the situation.

So to make it short, this is not tonsil cancer even the tonsil looks wierd and red of course you can not rule cancer further down the throat, but it would be unlikely as the sometimes pain in the lower throat comes and goes for years? It could also only be GERD reflux and post nasal drip related that mucus alerts my throat every night and causes my problems?

If I want to rule out lower throat cancer I have to do VL-Scopy? As I am unable to do that right now, I hope it is not cancer lower down so it will be too late in a month or two when I could have this done.

But the syptoms are not very consistent of throat cancer of any kind (just your guess of course). I am not sure how prevalent is throat cancer and laryngal cancer for a non smoking 51 year old.

Thank you

Hi,

Thank you for following up.

1. True, symptoms of cancer is progressive and will usually not get better nor wax and wane. The duration of your symptoms is also not compatible with existing cancer. The location of the pain is seen commonly with chronic GERD.

2. for the extremely rare possibility of missing something, I do a Video laryngoscopy (VL-Scopy). You can share images of a VL-Scopy here. This VL'Scopy also picks up clues to benign causes such as allergy, reflux, PND (Post Nasal Drip), and so on.

3. This is not guesswork, but an opinion from examining and being involved in the treatment of thousands and lakhs of patients over three decades. If a patient visits me for consultation saying he/she has cancer, I can assure you that more than 99% do not have cancer after a thorough investigation.

I hope that I have answered your query. If you have any further questions, I will be available to answer them.

Hi,

Thank you for following up.

1. True, symptoms of cancer is progressive and will usually not get better nor wax and wane. The duration of your symptoms is also not compatible with existing cancer. The location of the pain is seen commonly with chronic GERD.

Thank you very much. I still have the white/grey on tonsil, however I don't have pain with swallowing, I guess that rules cancer out even if the pain gets back again.

What do you think is that white thing on the tonsiles that never goes away completely?

This is my last question, then I close the discussion. Thank you for all your great help.

PS - That is interesting, more than 99% of cases who think they have cancer don't. In your practice, do you most often find cancer with the patients who don't think they have it when they visit you?

Brief Answer:
Wishing you good health.

Detailed Answer:
Hi,

Thank you for following up.

1. The white/grey areas are usually areas of patchy inflammation, chronic allergy or inflamed tonsillar crypts.

2. If you take a bland diet and routine medication, this will settle down in a few weeks.

3. There are always different types of patients, it is difficult to generalise. However, since tonsil cancer is rare the chances of diagnosis are equally rare. By thorough examination and investigations it is possible to rule out any form of cancer.

I hope that I have answered your query. If you have any further questions, I will be available to answer them.

Hi There I Need Help As I Am Scared To

Hi there I need help as I am scared to death about my symptoms. Thank you in advance for your answer Yours Sincerely XXXXXXX
