Question: Hi I have very high c peptide high insulin normal blood sugar. I am 135kg 55years old 5ft 6in I am not on any mess and I have not taken any in the past there is no history of diabetes in my family. I did have gestational diabetes with my 2 sons there test have have came back the same as mine one is 34 years old 243kg 5ft 10in the other on 23yrars old 143kg and 5ft 9in

Thanks for writing to us. Followings are my comments:

1) According to your information, we need to rule out INSULINOMA (tumor of the pancreas, secrets insulin from beta-cells) and MEN-1 syndrome in this context.

Their features are as below:

A) INSULINOMA > a) Low to normal blood sugar.
b) High C-peptide and Insulin level.

B) MEN-1 syndrome should be evaluated by high prolactin level (due to pituitary adenoma), Hyperparathyroidism due to parathyroid hyperplasia, Hypergastrinemia due to a gastrinoma. It needs to be excluded as D/D by some investigations.

2) INVESTIGATIONS: a) A 72-hour fast is gold standard to see if insulin levels fail to suppress, a strong indicator of the presence of an insulin-secreting tumor.

b) Some non-invasive imaging studies like ultrasound, CT scan or MRI may need to carry out to confirm the diagnosis. An Indium-111 pentetreotide scan is more sensitive than above tests.

c) Blood test like serum prolactin level, gastrin, calcium level etc should be done after consultation with your endrocrinologist.
