Brief Answer:
Infected hair follicle

Detailed Answer:
Hi

Thanks for posting the query

I have thoroughly examined the image you have attached with this query. I am of the opinion that you are having infection of the hair follicle.

As the skin of the ear cartilage is tightly adherent to the latter, any tiny swelling in that area would cause severe pain. The swelling you have noticed is an infected hair follicle.

My advice to you:
1. Avoid frequent touching of the ear tragus.
2. Topical application of a cream with a combination of an antibiotic and a steroid would help to relieve the inflammation as well as infection.
3. A good analgesic such as Diclofenac or Ibuprofen would be beneficial in relieving the inflammation and the pain as well.

Hope with the above medications you should get better. Revert back to me if you have any follow-up queries.

Thank you, doctor, for the thorough reply. If you allow me, I have one follow-up question. I read something about cholesteatoma. Could it be it? I do frequently touch that part of my ear, habit, so, I hope it is just an inflammation, but, I wonder, if it could be the above condition? Thank you.

Brief Answer:
Cholesteatoma is usually inside the canal and not outside

Detailed Answer:
Hi

Thanks for writing back

The Cholesteatoma is usually located inside the ear canal and not seen outside. In your case it is typical case of an infected hair follicle.

Hope this clarifies your doubts. Wish you good health.
