Hi, I Have Pain When I Pee And After Ejaculation

Question: Hi,
I have pain when I pee and after ejaculation since more than 1 week. I went to see a doctor few days after the symptoms appeared and she prescribed a broad range antibiotic course for 5 days. it got better but then became worst after the treatment ended.
Right after the antibiotic course, I tested for STDs. the results were negative for Chlamydia and Gonorhea (others STDs as well were negative).

So I went to see an Urologist who gave me antibiotics (injection and pills). We have re-made urine tests to culture the bacteria and I am waiting for the results (first examination shows that there is still an infection). There is no discharge from the penis.

What worries me today is that when I ejaculate thru masturbation, little semen come out since 2 days. Is it common with this type of condition and it is permanent?

Also, since I tested for STD right after my first antibiotic course. is it possible that I got false results because of the antibiotics?
Thanks a lot for your answer,

Brief Answer:
tests reports?

Detailed Answer:
Hello,

you haven't uploaded the test reports and you haven't told us the antibiotic you've used. What was the diagnosis initially? Did you see a urologist or your primary care physician?

Your symptoms suggest a urinary tract infection probably involving the prostate and perhaps also other structures of the lower urinary tract. If STDs are excluded then treatment with a quinolone is the usual first option but not for 5 days only. Depending on the suspected cause you may require treatment even for a few weeks.

I suppose the decrease in sperm quantity can be attributed to frequent ejaculations. Waiting for a few days before ejaculating should bring the volume of the sperm back to its usual levels. If this is not the case then you should talk to a urologist.

Taking antibiotics may make some tests appear falsely negative (particularly cultures) but I can't say what happened in your case. I don't even know exactly which tests you've done...

Since you've had recurring symptoms and you've already received some treatment, you'd better visit a urologist for examination and appropriate testing. If you'd like to upload your test reports I'll be glad to comment further on your case.

Kind Regards,

Hi doctor,

I have uploaded the lab results in the conversation.

1. The first results were made at Bangkok Hospital on the first diagnosis. Then I received a course of Cefditoren 200mg - 2 times a day for 5 days.

2. When the symptoms came back 8 days later. I went to test for STD, I uploaded the report in the conversation. Everything is negative.

3. Then I went back to see a Urologist on 22 July. He made a new urine test (i don't have the results of the culture yet but the WBC levels are still high)

I got an antibiotic injection (don't know which one) and now I have to take Prostagutt 160mg/120mg 2 times per day and Ciprobay 500mg 2 times per day for 5 days again.

Brief Answer:
antibiotic treatment is needed

Detailed Answer:
Hello and thanks for the additional information!

Your urinalysis suggests a urinary tract infection (and your symptoms as well). A urine culture may help to identify the pathogen, although prior antibiotic use may cause false negative culture results. You've done tests for syphilis and chlamydia and they're negative. I didn't see any tests for other pathogens like Neisseria gonorrhoeae or Mycoplasma hominis.

I suppose the injected antibiotic should have been ceftriaxone but you should confirm it with your doctor.

Ciprofloxacin sounds like a good choice (although I might have opted for levofloxacin in most cases) but I believe that 5 days is not enough.

You should discuss this issue with a urologist if you haven't done so yet.

Best wishes for a speedy recovery!

Can a urinary tract infection cause the ejaculation issue? (small amount of sperm released - almost nothing, never happened to me in my life) . If yes, would it resolves after the condition is being treated?

Thanks a lot,

Brief Answer:
probably not due to the infection

Detailed Answer:
Although - in theory - a urinary tract infection may affect sperm quality and quantity - depending on the involved part of the urinary tract - I do not believe that a recent infection under treatment has caused that damage. Have you given some time to your body to produce enough sperm? If you've ejaculated a few hours before then you may have a smaller amount of sperm the next time you ejaculate.

If it's due to the infection then the outcome cannot be predicted but it's not common to acquire serious problems after urinary tract infections. The infection will have to be treated first and then evaluate any progress to the sperm production. I'm sure eveything will be fine.

Best Regards!
