Question: Hi. I have had two minor cases of food poisoning in the past, and one major. The major was accompanied by one instance of vomiting, moderate fever, and the other usual symptoms. In all previous cases I was much better 48 hours later. Now, this is the fourth day, and my stomach still hurts when I eat the most modest of foods, even soup and bare bread. I have now gone and bought anti-diarrheal (loperamide HCL, 2 mg) medicine, and antacid/antigas (Aluminum hydroxide, 400 mg; Magnesium hydroxide, 400 mg; simethicone, 40 mg), but it barely has any effect. Especially, my stomach feels crampy, not burning, like heart-burn. I even felt, for the first time ever, a pain in my upper chest. Later, I had very severe gas, even after medicine. The pain in my chest went away. I was pre-med and went to grad school for medicine, but have no insurance now. I thought I had food poisoning from a special rice (wheat berry) that I made that sat a week in the fridge, but it did NOT smell. I tested my self by eating a tuna fish sandwich for breakfast on "day 1", and felt great and fine. I had the wheat berry rice for lunch and two sausage sandwiches, and 2-3 hours later I had severe diarrhea, and the rest of the night. The next morning, "day 2", I had a tuna fish sandwich for breakfast, and felt great. For lunch, I had the wheat berry "rice", with no sausage sandwiches, and 2-3 hours later I had very SEVERE diarrhea and gas for the next few days, without end, even vomiting the next day. I have barely been able to eat anything, I vomited after having two plain pieces of bread on "day 3". Even water or soup gives me cramps. I have a roommate who is a 7th grade school teacher, but we are not intimate, nor share meals, nor dishes, nor bathrooms. I have basically been made incapacitated. This is "day 5". I reluctantly went to the store on day 4 to buy more soup and the above medicine, but it barely helps.

Very likely you are having acute gastroenteritis but its not viral rather is bacterial in your case. Clostridium associated food poisoning is seen in cases related to rice and flesh.These bacteria's cause fermentation of food which causes inflammation of intestines hence causing pain and release large amount of gas causing bloating belching.

You have to add an antibiotic and anti protozoal to the medicines you are on(unfortunately they are not over the counter)
Tab Ofloxacin 200mg twice daily
Tab Metronidazole 400 mg thrice daily.
Don't take loperamide.It causes constipation and hence leads to stagnation of bacteria's inside your intestines.Rather try Racicadotryl 100mg three times a day(anti secretory agent).
Drink plenty of ORS or Electral.

In case your pulse rate rises above 110/minute or blood pressure systolic is below 100mm of hg go to the nearest emergency room.
Gastroenteritis should be taken very seriously.Patient can land up in shock otherwise.

You can visit any physician and get a prescription for the drugs mentioned by me.

Follow up after taking two doses of medicines.
