Question: Hi, I have had 3 sinus surgeries in the past 6 years; I ended up with psuedo aeruginosa , which is being treated by tobrymicin in a nebulizer for a year now since my last surgery. But before and ongoing still is severe ear/ETD pain ; i have had tubes put in , but the only relief I can get is to be on an antibiotic until it subsides. I have used Eustachi & all manners of ear excercises, chiropractor, and massage therapy to no long lasting relief & I do not have TMJ. Please advise; this completely takes over my life! Thank you

Brief Answer:
You can take tobramycin Nebulization for longer time.

Detailed Answer:
Thanks for your question on Ask a doctor forum.
I can understand your concern.
I have gone through the detailed history you have given.
In my opinion, you should take tobramycin Nebulization for longer duration.
Pseudomonas is chronic infection.
It stays in the body for longer time.
So you need to take atleast 3-4 months of inhaled tobramycin.
Please let me know
1. How long have you taken this Nebulization?
2. Do you smoke or take alcohol?
Please reply me answers of above asked questions, so that I can guide you better. I will be happy to help you further. Wish you good health. Thanks.

Thank you for your prompt reply. I had my last sinus surgery Aug 25 2017 & have been on the tobramycin for 10 months ; the last 2 months I have had bactoban with the tobramycin in my nebulizer , along with oral antibiotic of either z-pack or avalox in addition to the nebulizer treatments! If I stop the antibiotic for 3-4 days , the debilitating pain comes back viciously until I start back on oral antibiotic. I have tried taking steroids with pain medication without antibiotic , but NOTHING takes the pain, (so debilitating that I truly cannot concentrate or think coherently), nausea away until I start back on the oral antibiotic. I do NOT smoke or drink alcohol. I am on an anti-inflammatory diet because I also have SIBO. I consume lots of probiotics and drink lots of water. Is there any kind of pain medicine I can put in my nebulizer or in a nasal spray that would help reach the pain where it is?

Brief Answer:
You can use Lignocaine spray.

Detailed Answer:
Thanks for your follow up question on Ask a doctor forum.
I can understand your concern.
First of all sorry for late reply as I was busy.
You can try Lignocaine spray but I am not sure it will available without prescription or not.
You can also try centrally acting (acts on brain) painkillers like tramadol or morphine.
Hope I have solved your query. I will be happy to help you further. Wish you good health. Thanks.
