Question: Hi. I have been suffering some considerable pain recently in the anal area. I had a colonoscopy in July which was clear (a benign polyp was removed). There is no bleeding of late in the stools just the anal pain. I have used Uniroid-HC ointment twice a day (hydrocortisone, cinchocaine hydrochloride). My Dr has undertaken examination and advised area looks sore on the outside. It is also a little painful inside. Likely to be a deep anal fissure. I have now been prescribed lactulose liquid and Glyceral Trinitrate 0.4% recital ointment to be applied every 12 hours for up to 8 weeks. Is this better than the hydrocortisone? I am just worried about the side effects of this ointment - headaches, potential lowering of blood pressure etc. What would you recommend as a specialist in this area?

I am not sure what is causing this pain. Maybe following the colonoscopy procedure it caused some damage but it is uncomfortable when I pass stools so maybe hard stools? I do not have anal sex.

I would welcome your advice.

As per the reports of colonoscopy you are having fissure in anal area. Fissure in lay man's language just means a breach in mucosal lining of the rectal and anal canal.
As per your query regarding hydrocortisone is concerned oral use of steroids causes side effects however local application directly over the area of problem is associated with minimal or no side effects.

As per the best combination is concerned instead of using plain hydrocort use a combination containing contains Calcium Dobesilate, Hydrocortisone, Lignocaine, and Zinc. It will provide much better relief marketed as brand name Ruticool(Ask your pharmacist to provide you with this combination).

Secondly use a laxative daily for few weeks for easy passage of stools.

Add Isabghul husk to your regimen. It's a natural laxative containing husk. It absorbs water from the intestine and swells up making passage of stool easy and add bulk to stool.As stools would pass from the intestines easily breech in mucosa would be healed very easily and would not cause irritation again and again.

However, if not relieved even after 3 months of aggressive therapy surgical procedure could be done.Its highly effective.

Strict lifestyle modification should be followed along with medications-

Take simple organic and vegetarian home cooked low-fat food for few days.
Avoid alcohol, smoking aerated beverages, junk food like pizza burger etc.

Take lots of probiotic rich food like yogurt.
Increase your fiber intake take oats, wheat, barley, vegetable and fruit like Pear, apple, and oranges.

No aerated beverages.
Take fiber containing easily digestible food.
Avoid red meat.
Drink a glass of lukewarm water before going to the washroom in morning.

Thank you Dr. So is a hydrocortisone ointment more effective to heal this fissure than a Glyceral Trinitrate 0.4% ointment?

I was looking for Ruticool but can’t see it online. Is this a product available in the UK? Thank you.

No the product is not available in UK by same trade name but I have mentioned the list of ingredients it contain you could ask your pharmacist for the same.Trade names are different every where.You need to ask your local doctor or pharmacist for the same.

Yes hydrocortisone locally is more effective as per my experience with minimal side effect.
