Question: Hi

I have a soreness in the right hand to the right of the ring finger in the palm area. When pressing the area, the round bulb in the area appears to be significantly larger than in the left hand. In the left hand there isn't really a bulging round mass I'm the area.

The area can become a little sore with a dull ache extending up the ring and middle fingers. I have attached a picture with the relevant area circled.

The area is sometimes in contact with the edge of the hand phone. Is there a cause for the bulge?

I have seen the attached image and the area you mentioned. The pain in the palm can be due to a minor joint inflammation. The bulging mass also suggests mild tendon or joint inflammation. This may not be related to contact with XXXXXXX phone. Avoid any painful activities. Taking mild pain killer like ibuprofen and doing a hot compress in the area will help.

The joint and tendon inflammation can be due to tenosynovitis. This inflammation can be related to excessive use of computer mouse. This is not related to raised levels of uric acid.
The treatment includes oral anti inflammatory pain killers. In case the inflammation is persistent then local steroid injection under the guidance of your orthopedician will help.

I have seen a different GP who has been able to feel the mass and he is recommending Indocid medication. However, I have read the medication has quite a lot of side effects.

X-ray was recommended as a first step which I will do tomorrow. CT scan was said to be better for imaging the bone in case the x ray shows it is more the bone or joint inflamed instead of soft tissue such as the tendon.

'There is mild thickening of the A1 pulley in the volar aspect of the right fourth flexor tendon. There is a 10mm maximum dimension region of cystic degeneration seen further laterally. There is no restriction in flexor tendon excursion during passive and active movements. No other abnormality is seen.


COMMENT
Cystic degeneration and ganglion formation in the lateral aspect of the A1 pulley at the level of the fourth metacarpal neck. There are no signs to suggest stenosing flexor tenosynovitis. There is no evidence to suggest tendinopathy.


Based on these findings, what might be possible treatments or should I just leave it as it is? Is there any possibility that cysts or ganglions can become infected?

As per the report there is a ganglion cyst in the area. It usually does not get infected. In case it is not much painful then you can leave it as it is.
In case the cyst is painful then a local steroid injection at the site of the cyst can be helpful.
