Question: Hi I had a few concerns. I have been having pain inside my penis for about a week and a half now. It's a burning inflammed sensation that usually starts after I urinate sometimes when I urinate if I hold my pee for a while are just inflammed just sitting are while I'm at work the feeling goes up and down. I been put on bactrim last friday for 7 days I have two days left no relief I have had this type of issue for a few years on and off last year I got a mri of the urinary everything was ok including prostate 2 years ago I had a scope down my urethra it was clear idk what to do I go back to urologist in 2 weeks does this sound like chronic prostatitis? I have no discharge even before the medicine no lesions no redness are anything on my private area my girl has genital herpes but we use protection and we never have sex during outbreaks and she hasn't had that in abt a year we been together 8 and I never had any lesions are go unprotected so i wouldn't think that would be the case because wouldnt I have lesions outside of my penis if just say I had it inside my urethra? But I know we always use protection that was just an fyi question please help me

I have gone carefully through your query and understand your concerns. Going by the description it seems that you have developed urinary tract infection (UTI). You need to get your urine tested for routine microscopy and culture sensitivity. The infection could extend posteriorly resulting in prostatitis although that is less likely. We can look for it after the urine sample tests positive for UTI. However, that is not needed since the treatment for both conditions would be with appropriate antibiotics. It is possible that the infection is not sensitive to Bactrim and therefore you are getting no response despite taking it for a week. I would suggest escalating to ofloxacin or more preferably nitrofurantoin since these are more effective in UTIs. The choice can be re-evaluated after the culture sensitivity reports are available.

You also need to drink plenty of water.
