Question: Hi. I am a 56 year old man. i have had chronic low red blood cell count and hemoglobin last 4-5 years. 12.5 hemoglobin and 3.96 red blood count on my last testing result in August. My hemoglobin levels have been fluctuating last 4 to 5 years. 12.5-13.5 for hemoglobin. 3.96 to 4.10 for RBC. Iron counts and stores have all been normal. So no iron deficiency anemia. All other CBC readings have been normal except for my red blood reading

I recently had a colonoscopy and upper scope (August 2020) completed and no source of bleeding was found. They did remove a 3mm polyp in my ascending colon and i had second degree hemorrhoids. One other finding was Diverticulosis of large intestine without perforation or abscess without bleeding.

I also completed a Barium Upper GI series with small bowel follow through in September and nothing abnormal was found. Except a small hiatal hernia. Nothing abnormal was found in my small bowel on the x-rays

I took a FIT stool test today and it was positive. I have been talking iron supplements and many other supplements so not sure if that would cause the positive result. I don't know what to do now. i have had upper, lower scopes done recently (august) and the small bowel Barium follow-through. but no source of bleeding was found.

What do you recommend as a follow-up? Tied of taking some many tests but concerned of the positive FIT stool test. Could i have colon cancer that was missed during my upper, lower scope examines or small bowel study?

You had a colonoscopy in August which did not show anything suggestive of Colon cancer. So it is unlikely you have colon cancer.
As you said FIT stool test result must be a false positive one.
You can repeat it after avoiding the supplements, red meat, turnips, broccoli for 3 days.

So there is a possibility that the FIT test could be a false positive? My bowel movements have been very dark. I think the color is dark brown that is why I decided to take the FIT home test

I have heard about a GI bleed scan, nuclear medicine. To see if bleeding is going on somewhere in my digestive track. That wasn’t found on my colonoscopy and upper scope. Would you recommend this test?

Finally, what might explain my fluctuating anemia?

Hi,
If there is progressive drop in hemoglobin and FIBT is positive after taking all precautions and the upper and lower GI endoscopy are normal, then you can go for GI bleeding scintigraphy.

Your hemoglobin is fluctuating around the lower limit of normal range only and not to be worried about.
