Question: Hi,
I am 40 yr male.My blood sugar levels are high from past few years.Recently I got my blood test done and consulted a doctor.
On 1st Sept 18-My blood sugar fasting was 161mg/DL,post prandial 244, platelet count 1.43 lk/cumm.
Doctor advised Galvus met (50/500) 1 tab before bfast and before dinner.
Blood test redone on 16th Sept 18: Fasting blood sugar 88, post prandial 123. However platelet count reduced to 69 thousand..
I stopped the medicine and redid the blood test after 2 weeks.
On 30th sept 18; Sugar fasting- 163, platelet count 98 thousand.
Based on doctor advice changed the medicine to amaryl 1mg before breakfast.
on 7th Oct blood sugar levels (fasting) is 130. Platelets counts gone down to 95 thousand.

Kindly advise what to do for platelet levels to increase and control sugar levels.

Brief Answer:
Suggestions to add folate

Detailed Answer:
Hello.
I have read your message.
I think I can help you.
I would suggest that you add folate 5 mg once daily to your therapy and repeat the test after 7 to 10 days.
Usually a folate deficiency causes the levels to fall and minor metabolic adjustment caused by the medicines can cause platelet count to fall .
So additional folate supplements may help.
I have tried to make it as simple as possible.
Best of luck.

Hello. Thanks for your response.
Yes. I would recommend that you continue your sugar medication.
Do monitor your platelet count to be sure that the count doesn't fall to a dangerous level while the folate acts.
I hope that this information is helpful.
Best of luck.

Dear Doctor
I got my folic acid test done today. The folic acid level is >20 ng/ml. Vitamin B12 is 205 pg/ml. Calcium is 9.6 ml/dl.
I had just taken 1 tablet of fallate 5mg yesterday night.

Do i continue the medicine as the folic acid is above limits. What shall i do for the low platelet count?

Detailed Answer:
Hello.
Apologies for the delay.
I would suggest continuing both, vitamin B and folate.
If at the end of 1 week of these medicines, you continue to have low platelet count, the medicines will need to be changed.

I would suggest starting only metformin in that case and watching the platelet count and sugars thereafter.
Best of luck. Dr Mittal
