Question: Hi, I am 27, female, weigh 49 kgs and am having lower back pains during my period, also a little before the start of period and after the end of it. This is pretty new for me, as I've not had this pain during my period before. It causes general discomfort and more while standing or sitting for longer hours. I have a minor vitamin d deficiency, and do eat adult Vitamin d gummies. My maternal family has cases of accelerated osteoporosis, so should I be worried? Or are there any remediations?

Hello, I'm Dr. Branch, thanks for using Ask a Doctor. Cramping and pain, including lower back pain, is a common complaint with menstrual periods and is called dysmenorrhea. However, in a woman who begins having this kind of pain after the age of 25, this suggests that there is a secondary cause for the symptoms. Usually this will involve something with the female reproductive tract such as the uterus or ovaries. Possibilities include pelvic inflammatory disease, fibroids, or adenomyosis. The only way to diagnose one of these would be to get a pelvic exam and probably an ultrasound to look for these abnormalities.

Usually the initial treatment for dysmenorrhea is anti-inflammatories such as ibuprofen. If you have pelvic inflammatory disease, this would need to be treated with antibiotics. The other causes, if severe enough, may require treatment with some form of birth control. Rarely, patients would need surgery for symptoms that are not being controlled with medical treatment.
