Question: Hi Dr. XXXXXXX
Here are the my daughters CBC and WBC report about 2 months before we got the HIV Antibody test done.
Here are my 10 year old childs blood test results.

One is WBC Differential and the other
is CBC w/Differential.

Few questions Dr. Neutophils came a little low.
Lymphocytes came a little high than the range.

I have attached the report.
Earlier you were mentioning that
Only in negligible cases, individuals can show absence of antibodies in very advanced stages of immune suppression where new antibody formation would not be made by B lymphocytes which are also affected by immunosuppression (Liver and spleen also will be diseased). Antibodies will wean in the absence of antigens in the body. Antibodies are also not persist for years, but reproduced on stimulation by the memory cells like T4 lymphocytes.

So two questions here Dr. XXXXXXX

1) Do you see or suspect any immunodeficiency after looking at the report? I can see those three values(neutrophils, lymphocytes and the RDW came slightly out of range. I might have missed something in the report.

I am not a doctor but i remember the lymphocyte word
So when i saw the lymphocytes value slightly out of range i was worried. I thought i will ask you.

2) Is there anything i should be concerned about here?

Brief Answer:
No sign of cell mediated immune deficiency

Welcome once again.
First of all, the complete blood cell count and values will contribute as clues and not an absolute indicator of any diseases or conditions.
Your daughter's report were well within normal level only.
HIV the immune deficiency is of cell mediated type.
The counts will be affected only after established immune deficiency, not immediately.
In case of immune deficiency due to HIV, Total WBC count may likely to go below 4000 cells/ cu.mm and in the cell mediated immune deficiency, the lymphocyte count will be decreased to less than 25% whereas in your daughter's case it was raised than normal. So there is no indication of immune deficiency at all.
The variations in the values of neutrophils and lymphocytes can occur due to recent infection which had come across successfully by the body other than HIV.

and again the Lymphocytes, Absolute is still in the range.

And so i do not need to worry about the HIV Antibody test being a false negative correct Dr?

Lymphocytes, Absolute 3.37 thousand/mm3 1.30 - 6.50 thousand/mm3.
1) So is there any immunodeficiency here at all per this report correct Dr?
2) Do they point to any disease or medical issues? Are the CBC w. differential and WBC differential reports normal?
3) Is the HIV antibody test accurate or conclusive and not a false negative? Since there is no immunodeficiency i am assuming it that it cannot be a false negative.
I am just hoping and praying all is ok.

Hi,
Welcome back.
1. There is no evidence for cell mediated immune deficiency in her blood reports.
2. Complete Blood count itself is not an indication for any disease, but will support the other reports and clinical examination. Minor variations in the blood count can occur then and there as per the mild variations in the health condition of the body.
3. False negative HIV test is unlikely after the window period, especially with multiple reports.
