Question: Hi Dr. Saghafi,

It's me again. I wanted to find out how long the side effects will last. I stopped the Paxil and Strattera 3 weeks ago? I am still experiencing side effects and made an appointment with a doctor to have my blood work done. All vitals were fine, blood pressure 96/60 should this be a concern? I have always been in this range.

Thanks for reaching out and I'm sorry you're not feeling better by stopping or not continuing your medications of the combination Paxil/Strattera.

I went back and reread the other consult we worked on and so I know that you had fallen in the bathroom and really clocked yourself on the toilet and then, the wall? So, now with your ongoing symptoms which you are thinking are continuing side effects.....I have to ask a question as to whether or not you could've suffered a CONCUSSION in the bathroom when you first went down and if THAT may not be the reason you could be feeling the way you're feeling?

So, how about a description of your actual symptoms as we are speaking today? Dizziness, lightheadedness, any headaches, etc. etc. Your turn...

As far as the washout period is concerned for something like Strattera it kind of depends on how long you were taking the medication, and what dose you were taking. If you only took a single dose so that it was literally "one and done"....then, I would say it should've washed out of your body long ago. Even it combined with the Paxil....I would not expect any drug to drug interaction to be lasting for 2 weeks or how ever long it's been.

If on the other hand you'd been taking Strattera for more than say a few weeks and let's say you were on a higher dose of some sort...well, then, probably most doctors would say that maybe it'll take anywhere from 2-4 weeks to completely clear out...although again, from a purely pharmacological point of view....by now I would've expected the majority of the medication to be out of your system to the point where if we did a regular blood assay looking for levels...they'd literally be undetectable.

If you want actual numbers and I were to shoot on the high side of how long it takes to metabolize out of your body (assuming you were what they refer to as a POOR METABOLIZER) then, SIDE EFFECTS should cease within about 95 hrs. from the last dose of medication. A more likely washout if you had only 1 or just a couple of doses of medication would be along the lines of 24-36 hrs. Close to 2 weeks is just not possible....so we need to consider something else.....read on lass.....

Therefore, any symptoms you are still feeling COULD BE PART of a POSTCONCUSSIVE SYNDROME or PCS.

I don't know exactly what all lab tests they want to run but depending on what symptoms you're complaining of my dear...I would think in terms of a concussion followed by symptoms which could last from a few weeks to a few months....I know that's not very convenient....but it's better than being laid up in a hospital with a fractured orbit which Mr. Porcelain could've caused you....so there's the bright side! LOL....sorry... ;)

So as I said up top, your turn....tell me your symptoms....and whether or not you felt dazed, disoriented or otherwise, messed up in your head when you fell and smacked Mr. Porcelain....do you have a bruise over the orbit that you've been taking care of with something just so people at work wouldn't think the WRONG THINGS about what's going on at home???

Any lacerations on the scalp or head? Any dents or holes in the bathroom wall to show how hard you may have struck the wall...and also what part of the head got struck?

96/60 is really a very good blood pressure and especially if you know you've been running that in the past anyways....I'd say you've nothing to worry about there....

BTW, good hydration, eating 3 square meals a day...free from junk food and other garbage that we typically eat as Americans, and good exercise routines would be helpful....unless someone examines you and finds a reason for you not to be physically active due to balance or equilibrium issues which could be problematic if you're really suffering from a PCS.

Ok....young lady....as one of my good friends from high school always said back in the day, 'NEXXXXXXT!" I leave it to your imagination as to what he might've meant back in those teenage days.....while what I mean to say is...let's hear you answer some of those questions up top! HAHAHA!

And BTW, the Headache Summit for the AHS is scheduled for Nov. 22 this year...which is kind of late and UNFORTUNATELY, I'm not sure I can make it....we have a dress rehearsal for our annual MESSIAH CONCERT on Nov. 24th here in XXXXXXX which means I would have to catch a Red Eye out of XXXXXXX Sunday morning and of course, in the transition back to this side of the country we lose...what is it, 2 or 3 hours? So there's probably almost no way I'd make rehearsal unless it were a non-stop flight....The good news is that I'd have my violin with me anyways and it's only a dress so I can basically run in "as is" and start playing....still that's a mammoth piece to play through non-spot after being on a plane for 4-5 hrs. straight!

So we'll have to see....of course, you could always come up this way...even SOONER than Nov. 22 if the neurologists down there are tied up too much to help your doctor figure out what's going on....

Let's see, you've got some top notch folks at both the Mayo Clinic in XXXXXXX and the Barrows Institute....

Whatever it takes....I do hope you start feeling better....hydrate, hydrate, hydrate down in that desert....don't get hypoglycemic, and see if you can't get a bit of exercise....all of that may make you feel better.
