Hi Dr. My Nephew Has Developed Pain In His Right

Question: Hi Dr. My nephew has developed pain in his right thigh and faces difficulty in walking(Pic of thigh attached) . PAIN is written on the pain area. It has not happened before. We took him to ortho doctor and Dr suggested MRI. MRI suggests marked soft tissue edema and inflammation of the right thigh having free fluid of 14X2 cm intramuscular collection(likely infective). Report is attached. Dr advised us to do chest x-ray, blood/urine culture and some other tests for TB, ESR, Peripheral smear etc. Reports attached. We are being told that to drain the free fluid , he will need to undergo surgery( incision/cut to drain/suck fluid). I think the report says UTI(ecoli>100,000), while the doctor told my sister that he doesn’t have UTI. Doctor hasn’t given us any antibiotic so far. I wanted to ask is it not possible to cure this by giving diuretic and antibiotic medicine or is surgery the only option? We are little doubtful on the treatment recommended hence wanted a second opinion. Is there any other way out apart from surgery? Please go through the reports and kindly share your valuable inputs.

Thank you for posting your query and uploading the reports and the image. I have read your query, and also seen the image and read the reports.

In my suggestion, surgery is the best option here. Your nephew has marked soft tissue edema and inflammation of the right thigh having free fluid of 14X2 cm intramuscular collection (likely infective) - As per the MRI findings.

This collection is quite large, 14 X 2 cm. It would not be possible to cure this by giving diuretic and antibiotic medicine alone. This collection has to be drained surgically as you mentioned. Diuretic is of no help here, and it is not required too. Antibiotics are definitely needed. The operating surgeon would definitely put your nephew on appropriate antibiotics before proceeding with the surgery.

Your nephew does have UTI since e coli is >100,000. The antibiotic that would be given prior to surgery would take care of this UTI too. So, do not worry.

Like I said, Surgery is the best option here. This would help your nephew suffer less pain. It would help him recover and heal faster. Also, he would be able to walk and get back to normal life again soon.

I do not think there is any other way out or there is any other better treatment plan for your nephew. I would suggest you go with the surgery.
