Question: Hi Doctor,

I had a protected sex with a prostitute, but i used my tongue in her vagina for about 5 seconds and i was kissing her lips for a 10 to 20 seconds.

I took HIV1 and 2 test at 29th day - its negative
took Rapid test at 33rd day - its negative
took RT PCR - Quantative test at 33rd day - Result is "Target not detected"

should i need to take any more tests

Welcome to HCM. I have gone through your query and understand the concern you are worried about.

You had protected vaginal sex with a prostitute but used your tongue for oral and kissing. The risk in your case is unlikely because oral sex is less risky as compared to unprotected vaginal or anal sex.

You had took HIV 1 & 2 at 29th day along with RNA RT quantitative and rapid test at 33rd day. All these are negative then nothing to worry at all because these are highly reliable and conclusive tests.

As such no further test is required but international HIV guideline says to get tested after 3 months as routine to declare some one 100% free from HIV. Calm down and go for the test without any tension.

Thanks for your response.

i got some symptoms like stomach upset from 3rd week onwards, till this day and throat sore on 5th week, sometimes i feel tires and feverish..... is it the symptoms of HIV. and i dont know is it because of my anxiety.......but i didn't had diereaa or high fever

also i have uploaded the PCR report, i am confused is it RNA report or any other normal PCR report, because its mentioned as "Quantative Real Time PCR".no where its mentioned as RNA.

I went to a general doctor here, he has given me Anxit for 5 days. can i take this for another 2 months as i feeling very much anxiety.

also shall i take ELISA this week(its 5 weeks and 3 days now from exposure)

Your symptoms are mostly related to some other viral infection or anxiety but not as to HIV so don't get too scared or tensed. Consult your physician, go for some of blood tests to isolate the cause and follow the treatment regime.

The report is for RNA PCR detecting the viral load and it is negative which indicates you are free from infection. As such no need to go for any test at present but still for your mental satisfaction you can surely take ELISA.

Allay the unwanted fear or anxiety related to HIV because the risk is not there in your case. Taking Anxit for 2 months is not advisable, better to do some yoga or meditation to relax your mind. Consider counselling services if needed.

Hi Doctor,

I took a ELISA test in 42 day(close to 6 weeks), results was "Non Reactive" and
I tool a HIV DUO test on 49th day (close to 7 week) and result was "Non Reactive".

I have attached the HIV DUO report for your reference.its mentioned as 4th generation test,

1.Is there any possibility that HIV can develop after 7 week and can be identified in 12th week. because i am searching in forum that they are saying that 6th week (4th generation) result can be conclusive, is it?

2.In HIV DUO report, i have seen a index value as (0.1), may i know what is that mean, is it mean that i have started infecting with HIV.Please make me clear on this, i am really confused.

3.If my symtoms are related to HIV, antibodies would have been detecteble in HIV duo test, am i correct.

The result of 4th generation HIV DUOs test after 6 weeks is definitely conclusive and it was negative along with negative ELISA at 6 weeks then nothing to worry at all because you are absolutely risk free.

For HIV DUOs test any value below one is taken as non-reactive so don't think much about index value because it has not of much significance.

You are absolutely right that if any HIV infection is there then HIV DUOs test will surely detect it which is not there in your case so calm down, be relaxed, move ahead in life and forget all about HIV.

Thanks a lot for response.

I am worried about my PCR test, sorry for asking this question again and again. what is the difference between RNA PCR test and Realtime Quantitative PCR - viral load test. i am seeing in other forums that HIV diagnosis done by RNA PCR. its confusing me that have i done the proper PCR test.

for past 2 weeks i started getting rashes and small bumps in my shoulder and in my back side, it worries me a lot and on 6th week, i had fever for 3 days, but it was never went more than 100 degree. doesn't took any paracetamol, it went after 4 to 5 days. now for past 2 days, i feeling cold in throat. it looks like all these symptoms are related to HIV...........

Hi Doctor,

I tested in Apollo diagnostics after 8th week and 9thweek of my exposure.
(HIV 1 and 2 Anti bodies - they tell this is ELISA (3RD GENERATION)

1.As of now I am not seeing rashes and I didn't feel fever for past 1 week. I got lymph nodes near my ears and my neck is paining. as far as I know these are HIV symptoms. am I getting seroconverted?

2.I got honey bee sting on Saturday morning and I started to observe pain in my neck from evening and I guess lymph nodes became painy afterwards. is it due to honey bee sting.

3.I am observing symptoms one by from 5th week, now its almost 10 week(66days).
please make me understand, if my symptoms are related to HIV, should I been tested positive my this time(atleast by 9th week (3rd generation test)).

I cant say I am observing this symptoms due to my anxiety, lymph nodes cant come due to anxiety.

Do you think is it because if EB virus or CMV virus?

I don't what I have to do............ I am almost gone my now.The results of 8th and 9th week is "Negative", I didn't mention that in my previous update

You had done 3rd generation ELISA after 8 and 9 weeks of exposure in Apollo diagnostics and both of then came back negative then you are free from HIV and the result is highly reliable so don't worry.

Pain in the neck can be due to honey bee sting for which take some pain killer and it will be fine soon. If you think this is due to lymph nodes then consult ENT specialist to isolate the cause and follow the treatment regime.

The possibility of HIV in you case is very unlikely or practically nil so be relaxed and move ahead in life. Don't get anxious or feared because it can give rise to many unrelated symtoms. If still any query left you can get back to me again any time.

Thanks for your response.

1.usually when the symptoms starts to appear for hiv. Because for me symptoms starts one by one from 5 th week and still going on.if one symptom goes then another one comes.

2.practically if my symptoms are hiv related then I should have tested positive by my 9th week test, am I correct?

3.as my pcr test was quantitative it may not be able to detect below 40 copies/ml, is it possible an infected person can have less than 40 copies after 1 month of exposure.

4.shall i repeat RNA detection PCR for HIV1 and 2 now since I am getting symptoms.

5.do you think in am infected with EB virus or CMV.

6. Do you think i need to get STI screening, but I dont have any issue in penis or urination as for now.

Yesterday I went to doctor here, he examined my whole body and he had a look into my penis as well. He think that there is something happening in my body. But he can't confirm that it's hiv since PCR was negative.

The symptoms of early HIV infection start after 15 days of infection but I don't think your symptoms are related to HIV. They are more so ever related to anxiety or fear of HIV because if infection is there then it must be detected by ELISA or PCR test.

HIV RNA PCR is able to detect the virus if infection is present and it is also very unlikely to have less than 40 copies after one month of exposure. If you want to get HIV PCR repeated definitely go for it as the negative result will clear all your doubts.

I don't think you are infected with EB virus or CMV. Also no need to go for STD screening as the risk is not there in your case. Do meditation or yoga for mental relaxation. Forget about the incidence and move ahead in life. Stay happy and healthy always.

I have done again an Elisa on 10th week(70th day), it turns negative.

Since I am getting engaged, I am thinking of doing another PCR tomorrow for HIV1 and 2 in 11th week (77 days), also I am continuously getting different kind of symptoms.

1.could you please let me know which PCR test should I need to take for HIV 1 and 2, just to remind you that I have taken QUANTITATIVE Real-time PCR at 33rd day mark which results as "Target not detected"

2.i was checking in Lister Metropolis LAB for these tests, below are the tests available. please let me know which TESTS should I need to take for confirmatory at this point of time.

Hi Doctor,

I actually given my PCR tests on 77th day, but lab called me on 81st day and told me that the sample is insufficient.I have given blood sample again on 81st day.they released the results in 2 days.

Results,
HIV1 - RNA detection using cobas method (limit of detection - 20 copies/ml) - Not Detectable.

HIV 2 - RT PCR QUALITATIVE (Limit of detection - 900 copies/ml) - Not Detected.

1.is there a chance they ran tests with my initial sample, but it would have given positive results, so they asked me to give it again.I asked the same question to metropolis lab, they denied it.

2.I took ELISA in Apollo and CMIA HIV DUO in lister on 90th day.both were negative.can I consider myself negative as I touched 90th day.

3.I went to dermatologist for my rashes, he told that it's definitely some viral infection. So I explained about my sex exposure and my HIV tests. So he asked Me to take complete set of STD panel test, I will get the results by next week only.

4.my current symptoms are rashes and i never got rashes in my life before like this.can this be HIV.

5.from my childhood I used to get mouth ulcer every month and I will lasts for 1 weeks and 7 out of 10 times I will get lymph nodes in my jaw when i am getting mouth ulcer.Is that means I am a person who is immuno suppressed since I am getting frequent lymph nodes when I am getting mouth ulcer.because I am reading in internet that immono suppressed person will get positive results after 6 months.now I have a painy mouth ulcer and painy lymph node in throat. But rarely get I'll in past.

Sometimes it happens that given blood sample is insufficient, it does not mean the initial test was positive and the lab people want to re-confirm as said rightly by metropolis lab. RNA PCR is negative for both HIV 1 & 2 so don't get worried.

ELISA and CMIA HIV Duos tests are 100% conclusive after 3 months that's why you can definitely consider yourself free from HIV so don't get anxious or tensed and forget all about HIV.

Skin rashes could be due to some other viral infection but not related to HIV for sure so be relaxed and wait for the STD panel test result as suggested by your dermatologist. The test will clear all your doubts and isolate the cause.

It is not possible to get sero-converted after 6 months even in immunologically compromised patient also so don't think too much and move ahead in life. For any further query feel free to ask and I am always here to help you any time.

As i checked it looks like I have Rubella and also HSV 1 and 2 IGM values are in bold, looks like i am positive for that as well.

Can rubella cause Rash and Small Lymph nodes.

I yet to collect my Chamydia report.

Is EB Virus test is needed for me?

Hello Doctor,

Sorry that i am continuously disturbing you, I have few more questions,

1.Can rubella be cured? i am getting engaged this week, can i continue with my plans

2.Can Rubella or EBV delay the HIV antibodies?

3.Is it possible my HIV tests would have disturbed by rubella virus or EBV, should i need re take any HIV tests again

4.I have checked for EB virus tests, there are multiple tests available, i have attached the screen shot of the same. can you suggested me the what are all the tests should i need to take, I am okay to take all if it is necessity.

5.Can you comment about my HSV 1 and 2 results, are they positive or not.

6.I did a complete blood test on 33rd of my exposure, by that time my lymphocyte was 26% and i did the same test on 88th day, on this time lymphocyte is 32.9% - can lymphocyte percentage increase this much(7%).

7.is Rubella an STD?

I am really really sorry for asking the questions again and again......... i meet with doctors, they are not ready to hear what i am telling about my symptoms.they go with only my HIV results.Finally my dermo doctor told me to go for list of STD tests, in that rubella came positive.

Once infected the treatment is only symptomatic for rubella.

Rubella can be prevented by the vaccine available for it named MMR vaccine which is given along with mumps and measles but once infected there is no specific treatment available.

Rubella and EBV virus has nothing to do with result of HIV testing and you can go for DNA PCR among the tests you had mentioned. No need to take any further test for HIV as rubella is not an STD.

HSV 1 & 2 are not positive in your case and rise in lymphocyte count is due to infection nothing else so don't worry about that. The treatment of rubella is only symptomatic but once recovered person is immune to rubella and will never get it again.

Consult your dermatologist and follow the advice because the risk for HIV is very unlikely so forget about it. You can definitely move ahead with your engagement plan after an expert advice.
