Question: Hi Doctor, I am 37 years old. I went to unprofessional massage centre today. There massage therapist was a female, who is doing massage for more than 5 years in different cities in Tamilnadu. My situation is she massaged me for 45 minutes and she did handjob for 5 minutes. No kissing and penetrative sex. I had a wound dried blood clot in leg finger, above the nails, which happen 2 days back. Assuming she is HIV positive and bleeding in her fingers and touched my leg fingers to massage. I saw her fingers and no visible bleeding in her fingers. Now I am worried and anxiety about my HIV transmission. I am married and have a kid.

Request you to answer my questions.

Does massage and hand job transmit HIV even if she is HIV positive?

Does HIV transmit through skin blood clot on my fingers? (Assuming she is HIV positive and bleeding in her fingers.)

Can I continue to have a sex with my wife?

There is every chance for the parlor girl could be a HIV negative individual. Even if we consider that she is a HIV positive individual, when she is on regular ART drugs, and if her viral load is undetectable, she will be no longer infectious. Odd possibility is that she had a fresh wound in her fingers. Even if we consider such a possibilty then also the possibility of HIV transmission is negligible only.
I don't find any reason to abstain from sex with your wife if all the statement mentioned by you are true.

Test with serum will be more accurate than with plasma as it contains more proteins which may cross react sometimes.
As far as I know there's no difference in window period with either plasma or serum.

1. If that friend is already on ART (Treatment for HIV) and if his viral load is undetecable, then the risk is almost nil
2. You have the risk of transmission of HIV, only if you have some ulcers in your mouth or some other Gum problems. Otherwise your risk is negligible.
When your friend is not HIV positive your HIV risk is almost 'NIL'.
You knew about your friend very well. Then why are you worrying?
