Question: Hi. About three or four days ago, the top of my butt crack started itching really bad, as if I’d gotten bitten by a bug. Then a couple days later, the area had minor pain and swelling. Now, I have a bruise the circumference of a golf ball in that area that is extremely sensitive (pain when sitting, bending over, laying on my back) and the bruise is turning purple. I’ve done research and saw many similar stories. It seems too large and off center (the bruise is mainly on my right buttcheek) to be a pilonidal cyst and I haven’t fallen and don’t spend an outrageous amount of time sitting for it to be a bruised coccyx so I’m out of guesses! Please help!

It's a little difficult to say for sure without being able to examine it more closely, but the appearance and your description of this sound most likely an abscess that is forming. This can start as a small infection of the hair follicle that then forms an abscess under the skin. This is a common location for them, and I believe the location makes it less likely to be due to a pilonidal cyst/abscess. These often have darkened skin due to the inflammation that can sometimes look like a bruise, and are also very painful.

The treatment for his would be incision and drainage, and possibly antibiotics, so I would recommend seeing a doctor for this to get it taken care of as soon as possible.
