Question: Hey doctor since Monday last week I have had this red rash on the left side of my shaft on my Penis. I was travelling overseas to Japan and on the second day I was there after having a shower I noticed the rash and it was quite red plus very itchy. The only thing I could think of was the hotel soap that I used, however I went to the pharmacy and they gave me an antifungal cream and it made it burn badly so the following day I visited a doctor to get it examined. He believed that the irritation was actually from using this new non latex condom I’ve been using the last month and it caused dermatitis to the area over the month so he prescribed me a very mild steroid cream of 0.5% strength which I used for about 5 days until I got back home 2 days ago. It has gotten better but it is still red especially after I have a shower. I then visited my GP to get further examined and she switched me over to Hydrozole cream which has a 1% steroid and also an antifungal cream bundle (I’ve attached images of them) in the same tube. I’ve been using this for 2 days now and the area is red but the initial itchiness has gone but I’m unsure if it’s making it any better. Should I also be applying a moisturiser to the area while using this cream because I’ve noticed a lot of white flaky areas as a result of using the cream and also when I roll back my foreskin with the glans there is a lot of small specs or smegma could this be from the steroid cream my doctor prescribed me Bactroban antibiotic ointment but I’m unsure if that’s the right thing to use for the glans? Just to clarify in terms of STD i have a girlfriend and only sleep with her so I know for a fact she doesn’t have anything so that rules that out. Do you believe I’m getting the proper treatment for this and if so how long would it take to improve?

Most likely you seem to have Intertrigo which is inflammation of body folds due to sweating and rubbing. There is often candida superinfection of the inflamed skin. In my view I suggest you to continue using hydrozole cream for 2 weeks. I would anyways like to have a look at the affected skin. You may upload a few clear images of the affected area. I don't think there is a need to use a moisturizer or bactroban ointment on that area.

Hey doctor so just a follow up I went to my doctor again after doing some tests and it turns on my glans of my Penis have a fungal infection but my shaft seems to be clear in terms of infection from the swabs she did I also was clear of any STD’s. So my doctor has got me to stop the Hydrozole since the redness has gone down quite a bit and she has switched me to the antifungal cream for both my glans and shaft twice daily. Now my question is in terms of the moisturiser can I use that on the shaft since there’s no infection there only the glans? I think the steroid just caused a couple of dry white patches would that help it and not make the fungal worse? In terms of the fungal infection my doctor said it wasn’t really a transferable fungal infection but she said to avoid sex with my partner until it clears up at what point do you think that would be once the symptoms go do you think the last time I had sex with my partner I used a condom as well which was last week. How exactly does one usually get a fungal infection I thought they were pretty rare in males?

You may use a moisturizer except if it is very hot and humid in your part of the world. As far as switching to a plain topical antifungal is concerned I absolutely agree with that. Since fungal infection is more common in sexual partners because of increased chances of transmission from skin to skin contact even though it is not classified as an STD, so better to get rid of it completely before engaging in sexual contact. I just had a question in terms of sex I’ve read that the Anti fungal cream that I am using can effect Latex condoms how long would it have that effect for after using it? Would it be safer to use a non latex condom for a while even after symptoms are gone and I’m still using the cream?

hi. I have viewed the images. The antifungal cream won't affect the latex condom. You need not change your condom. Of course you should take care that when you apply cream it needs to stay in for a few hours, therefore use the cream after you have had sex.

Hey doctor thanks for the reply Just a follow up I just had a couple of further query’s after visiting my GP.

So my gp has re examined the area and she said it has improved a lot since I last saw her the rash on the shaft has completely gone, the discharge from the glans is gone I just have a bit of redness left on the glans. However she has prescribed me a single one off dose of Fexofenadine 180mg I just had a few questions about it though. My GP said to take this tablet, stop the antifungal cream on both my glans and shaft then after 3 days to only apply the cream to the shaft to prevent re-infection and then not to worry about using it on the glans since it seems they are more sensitive. However with the tablet I’m little concerned as I have a very sensitive gut and antibiotics for example stir it up really bad and I start getting bad stomach pains could the antifungal cause a similar reaction? I’ve also read that it stays in your system for 7 days is that correct? She also explicitly said to not use antihistamines while using the tablet since they have a bad reaction why exactly is this? It’s also hay fever season at the moment and I tend to get bad hay fever as well so it’s a bit hard at what point after having the tablet could I take an antihistamine?
I’m going to keep using the cream for the next 2 days do you think that would be okay? Then I will see my GP about it again on Monday before using the tablet as I’m a little concerned about it and unclear on how it works.

Hey doctor sorry I meant to say Fluconazole my gp gave me the one off tablet to take for the week that’s my bad just a correction there so just replace that with what I said about the antihistamine.
