Question: Hey doctor,

So I’ve been had digestive issues for the last 4 years but in the last 6 months my symptoms changed to mainly consisting of upper right sided pain plus epigastric pain, nausea and reflux where as before it was mainly just an upset stomach and reflux. But now the reflux tastes different and PPIs don’t work and from what I’ve read it sounds like bile reflux however I just had an abdominal ultrasound and I just had a HIDA scan today and the initial results seem to be normal but I have to see my specialist to get the final results. Can you still have gallbladder inflammation even if the HIDA and ultrasounds are normal I have a family history or gallstones but I don’t have any. I’ve also had gastroscopy/colonoscopy and blood work which were also all normal so what else could be cause the pain? I’m getting a distinctive upper right side pain that’s sharp and I can feel it when I breathe in too which is triggered after eating and lingers around. Heat packs and hot showers seem to ease the pain is there anything else I can try medications wise and what should I be eating and avoiding food wise?

If I overeat and eat too much that makes my symptoms the worst more than anything else as well and overly fatty foods make everything 10x worse like a McDonald’s burger for example which I had this morning as they required me to consume a fatty meal before fasting for the HIDA scan. Even for dinner tonight I had one piece of grilled chicken, beans, rice, carrot and spinach and still felt sick especially the upper right sided pain.

Brief Answer:
You need to have very light food.

Detailed Answer:
Hello and thank you dear for choosing "Ask a Doctor" service for your query.
Have seen your details.
If HIDA scan is normal along with a normal ultrasound I don't think the culprit is gall bladder.
Gall bladder opens up in duodenum while reflux of acid takes place from stomach which is above doudenum. Hence bile reflux is much more uncommon then GERD and with a normal HIDA and USG it's almost not possible to have it.

Two basic diagnosis of upper right quadrant pain is either gall bladder disease( ruled out in your case) or Acid peptic disorder.
Please try to understand that PPI is just a baseline drug for treating GERD rather I would say that just giving PPI is a incomplete treatment in itself.

Why?
GERD (Gastro esophageal reflux disease)-
Acid secretion is controlled by PPI's but what about the second component Reflux?
No drug has been given to control it.
Either levosulpiride or Domperidome should be added to PPI's.
Secondly which PPI were you on and what was the dose?
For example doses of Esomeprazole ranges from 20 mg twice to 80 mg twice daily.
So unless and until patient stops responding to maximum doses one can't say that medicine is not effective.
Thirdly Magaldrate syrup to your regimen. A very good antacid.

Now let's talk about diet.
In control of acid peptic disease the biggest game changer is life style modification. Harsh but truth is that no amount of medication would help you if you won't have a diet which suits you.
For e.g you have got a lacerated wound on your hand...No doubt you apply wound healer daily but you scratch the wound twice a day..Would it heal?
Answer is no.
The same is true with acid peptic disorder.
On one hand you take medicine to suppress acid formation while on other hand you are taking food which is junk and needs lots of acid and other enzymes to digest it and even sticks to intestinal wall and acts as a breeding ground for yeast and bacteria. In such case medication won't help.
Solution-
Eat clean light food 70% of times
Rest 30% of times you could have cheat meals ( please don't binge).
Initially make it 80:20 then slowly and slowly try to bring it to 50:50 ratio(If you really can't control your junk requirements).

Secondly right now you need to detoxify your body.
Eat 50% food as raw( fruits nuts salads) and rest 50% as cooked.
Food should be very light low in fat and should be home cooked.
Take Kawa roots extracts for anxiety.
You can eat all vegetables.
If possible no flesh for few weeks.
Drink a cup of chilled milk(low fat) in morning.
Take some saffron and leave it overnight in water. Next morning drink that water. Saffron has excellent anti inflammatory property.

In case you want to know about herbal medicines ( effective ) please revert back. They are usually helpful in detoxification with a proper diet.
Hope I was helpful.
Please feel free to follow up.
Revert back with doses of PPI's you have tried.
If satisfied with answer do rate it.
Thank you and get well soon.

Hey doctor thanks for the follow up I’m going to try your diet recommendations and see how I go. In terms of PPI’s I ended up at the XXXXXXX dose of 80mg a day and the higher I went the worse it made me feel it just felt like I wasn’t digesting food at all. I think I will try not eat meat for a couple of weeks and just stick to your vegetable recommendations even chicken seems to cause pain at this point. In terms of natural remedies I’ve heard aloe Vera, slippery elm and DGL are good what are you thoughts on those?

With the PPI’s as well I haven’t been on them in over two years and haven’t tried them since they just didn’t seems to do any good for me personally would it be possible I had low stomach acid to begin with? When I first started having digestive issues 4 years ago now it was after being on doxycycline for about 2 months for acne and after that I started getting reflux and stomach pains could the antibiotics I had back then done some damage in my digestive tract or caused something like SIBO? I really find drinking Yakult seems to help

Avoid all English medicines for few weeks.

Detailed Answer:
Hello again,

Along with it take isabghul husk 10 gram three times a day with chilled milk.

Take a probiotic daily.
Leave non veg or any heavy food for 3-4 weeks.
Take saffron with boiled milk.
Drink plenty of water but not forcefully( Drink only when your body feels thirsty).
Don't over think.
Take Medhavati(Herbal available online two tablets a day to calm your nerves).
No alcohol,tea or coffee.
Eat lighty cooked vegetables.
Avoid too much dairy.
Yoghurt may be taken.
Try this for few weeks.
You would start feeling change.
Engage your self in breathing exercise.
10 minutes Anulom vilom pranayam( just type this on you tube and you would know how to do it).
10 minutes of Bhramari pranayam.
10 minutes of Kapal bhati pranayam.
just copy these names on you tube and procedure to do them is very easy.
Follow up after 21 days.

Thanks for that doc I’m going to follow your guidelines and let you know how I go! I was just curious however what your thoughts on SIBO were? My gastroenterologist seemed to believe only patients that have had abdominal surgery seem to get it but apparently 78% of people with IBS have SIBO but the study’s aren’t conclusive yet what do you think could that maybe also linked to why food feels like it sits in my stomach?

Brief Answer:
Food is stagnated because the complete digestive system needs rest for a few days

Detailed Answer:
Hello again,

As per your query regarding SIBO is concerned a simple breath test needs to be done. When we have technology then there is no sense in making wild guesses.

A breath test is a common test for diagnosing SIBO. Excess bacteria in the small intestine can lead to the release of the gases hydrogen and methane, which can be identified through a breath test. This test is noninvasive and if SIBO test is positive then Rifaximin 550 mg twice a day would be given for 14 days.

Not a big issue if you feel it could be SIBO go for a breath test.
Hope I was helpful.
Follow-ups are welcome.

Thank you for the follow up doc, I will definitely be doing the SIBO test to rule it out. The other thing I had a question about is Helicobacter pylori. Now I%E2%80%99ve been tested for it twice already in the past, once last year and once the year before both when I was having the same symptoms as now but they both came out negative. However I do travel sometiems and this yesr I did go to Japan a month ago for 10 days and these flare up of symptoms 2 weeks later after coming home. Could it be possible I caught it? I didn%E2%80%99t eat any raw food mainly just western food while I was there as well and always washed my hands. I just don%E2%80%99t want to repeat tests if there%E2%80%99s no point. Just to reiterate my symptoms are an ache in the epigastric/upper right side, nausea, reflux and burping after eating.

Detailed Answer:
Hello again,
If at earlier two occasions if you were H.pylori negative chances of your being H pylori positive this time just because if a small trip is almost negligible. Flare up of symptoms are due to change in food.Even food served by airlines are spicy and oily I hope there is no need to comment about restraunts.
Your symptoms are typical of acid peptic disorder (chronic in nature) you need a well balanced home cooked light food for few months with proper medications.
Already too much of testing has been done dear.
Follow my suggestion you would start feeling better soon.
It's not SIBO
It's not H.Pylori( though you can go for tests).

Hope I answered your query well.
