Question: Hey doctor

I have had this problem on and off for about a year now but I keep getting this burning pain when I urinate. I feel it kind of right at the very start of the stream and also at the tip of my penis. Now I never have been sexually active in my life yet so I don’t have any form of STD and I have had multiple urine samples and swaps of my penis which have all come up negative for any infection. What I find makes it worse though is masterbation and I am unsure if it’s the technique in which I do it is possibly the cause? Usually I would do it in the prone position in bed and I feel that may be putting to much pressure on my urthera could that be why possibly especially since I’ve had countless tests negative for infection? If I don’t masterbate for like 2 weeks I find the pain almost goes away but it’s hard to maintain those off periods for so long and as soon as I do it again it eventually comes back. I visited a urologist recently and he believed that it was nothing major and the whole idea of my masterbation technique wasn’t the cause of it so I am really unsure what to think. I do sometimes see spilt streams in my urine after I masterbate as well so I don’t know what’s happening.

Thanks for query .

Based on the facts that you have posted burning urination that you get mostly after masturbation is likely to be due to inflammation of Urethra (Urethritis) resulting from trivial injury to delicate urethra while masturbating in prone position .

In view of all the relevant tests being negative this is the most probable cause that can be linked to your issue .

Try to change the position while masturbating in supine position with griping the penis in fist of the hand and shaking your penis gently .Observe for any change in symptoms .

You need to take broad spectrum antibiotic like Cefixime along with anti inflammatory drug like Diclofenac twice daily along with topical antibiotic ointment like Neosporin twice daily at the tip of the penis..

Ensure to wash your genitals with warm water twice daily and immediately after masturbation .

Just an update I did try the supine position

I also do have a bit of mild phimosis, basically I can pull the foreskin back perfectly fine with no issues while I’m not erect but while I am erect I can pull it about half way down and it hurts a little when masterbating supine because of it is that possibly why I’ve grown up with this method? Ejaculation results from adequate penile stimulation .In prone position one gets adequate penile stimulation ,penis being pressed between bed and body which lacks in supine position and that is the reason you do not get ejaculation .
