Hello Doctor, My Husband Is Having Fever Going Upto 102.5

Question: Hello doctor,

My husband is having fever going upto 102.5 F.
It started yesterday evening when he started feeling cold and after some time he was shivering with fever. He is taking Dolo 650mg every 4-5 hours but fever is not coming down much. Even now he has shivering and cold sensation with body pain.

Please suggest keeping in mind the Dengue scare in XXXXXXX
Does he need any blood test?

Hi,

I would generally wait for 2-3 days before conducting any form of test. I understand the scare, but most common cause of fever in the community is due to common viral infection.

He should drink lots of water and stay hydrated. Continue dolo 650mg every 6th hourly; alternatively combination of aceclofenac + paracetamol like tab hifenac P twice day may be used. Fruits and vegetables will boost immune function and should help him.

If the high grade fever persists beyond 3 days or if he develops severe joint ache, vomiting / diarrhea consider visiting a doctor for investigations.

Hello doctor,

Thanks for your response.
We got some blood tests today(reports attached)
The platelet count is 76.0 thou/mm3, which seems very low.
Does that mean he has Dengue?
What should be the course of treatment? Please prescribe some medicines.

Hi,

Low platelet is a common feature with most viral as well as some bacterial infection. The report is also negative for widal (typhoid). But I did not see the dengue test results. It would help if you can upload it (dengue report).

The treatment is supportive - good fluid intake and paracetamol tablets (dolo) 650mg.

Is he still running fever?

Hi doctor,

We are waiting for Dengue test reports and are expecting to receive them by evening.
We got another Platelet count done and it has improved to 80000.
He does't have fever now but too much body pain is there.
He is not able to sleep at night or eat much.
He also vomited today.

Dengue serology suggestive of an infection...

Hi,

I reviewed the reports attached. A dengue antibody tests done on 4/7/2019 reveals positive IgM titer. It is suggestive of dengue infection.

The treatment will be supportive treatment - as discussed earlier. During the treatment platelet count and PCV are to be monitored especially if he is still running fever. May I know his current status - is he still having fever?

If the fever has subsided, it would be necessary to repeat platelet counts for 2-3 days to make sure there is sequential rise in the count.

I will await your response on his status to discuss further.
