Question: Hello doctor ,
I am 28 year male .
5 days before i visited spa center .
There a female gave me massage .
I have seen she washed her hand not very well . she gave me massage with oil in my back , both legs and around joints of thigh & body (not in the upper body frontside) .
After that in the heat of the moment she gave me a handjob with oil (only foreskin she touched , without condom ).
I requested her to show her hand , in her left hand there was two red spots , in one of which she had very tiny opening and she refused to show her right hand . (I worry she could have some syphillis hand lesions as she frequently deals with customers)
after massage i didn't taken proper bath .

Is there risk of syphillis and hiv or other STDs . I am very much worried , now it can also affect my wife ?
What should i do now ?

Brief Answer:
STI and HIV risk are negligible

Detailed Answer:
Hi,
Welcome to ASK A DOCTOR through HCM.
Thanks for posting your query.
Organisms causing STIs and HIV are highly fragile and cannot withstand the outside atmosphere not more than few seconds. So transmission through casual contacts like touching is a remote possibility.
Syphilitic ulcer over hands are extremely rare. Other type pf syphilitic rashes over hands without any generalized lesions are also uncommon. So nothing to worry much.
As it had already been beyond 72 hours, there is no role for post exposure prophylaxis (PEP) for HIV in your case.
If you are still worrying much then you have to go for tests for STI panel and for HIV. The window period for HIV infection is 4-8 weeks with normal test. If you want, you can go for certain special tests, those will be reliable after 4 weeks.
Till you rule out STIs and HIV, it is better to avoid unprotected sex with your spouse.

Brief Answer:
HIV risk is negligible

Detailed Answer:
Hi,
Welcome back.
1. If you want to have test after 1 moth go for 4th generation combo test for HIV and RPR test for syphilis. Other tests for other STIs are optional. After 3 months, you can go for ordinary HIV rapid test is enough. Test for Hepatitis B and C can also be done.
2. Previous tests confirms that you are free from HIV/STD
3. Syphilis sore is an ulcer whereas pimple is just a papule.
4. Hand job or masturbation is the safest sexual activity free from risks. Body massage with oil also not have any risk.
