Question: Hello,

This is Anupriya here. I am trying to reach out to you regarding the situation I am in right now. I am having constant head pain for many days. It started from hip right pain on 2017 and then it all radiated to my right leg. During September, 2017 I was in such a condition that I was not able to get up from bed. I have been different doctor from Primary care to Gastroentologist to Rheumatologist nobody was able to say I am feeling like this. I also visited a Neurologist back then she gave me Gabapentin and let me go. My entire right side is in pain, my right abdomen, right hip, right part of my head. These days head pain are such that I am unable to sleep, unable to concentrate on things. Above all I am loosing my weight unconditionally from 145 lbs in November 2018 to 122 lb today.

My entire right side feels heavy and body seems to be parted with left side completely normal and right side heavy. I went to the Hospital and they did a CT scan of my head and neck and told me everything is normal, they asked me to see a Neurologist. So I visited a Neurologist and he did some test on me and told me my right side is not behaving properly, he has mentioned something called "Basal Ganglia" has asked me to EEG I did Brain MRI earlier in 2018 which was also normal, Neurologist whom I am seeing now is asking for that record. He wants to look for some tissue under a microscope then decide if he needs to do another MRI with gadoleum. He is also interested to perform some test which requires to sleep for 3 hrs to get some tissue from the brain. He thinks there might some poison or virus or some bacteria. All these things sound so scary to me.

Kindly advise if I am on right track, I am suffering for so long and I need to get cured asap. I am loosing the essence of my life as I am very bubbly full of life kind of a person and nowadays I hate looking at myself in a mirror due to these weight loss. what is this basal ganglia, is it curable can this cause weight loss like I am having, please advise.

First of all I like to clarify that I am not a neurologist but a neuro psychiatrist, a specialist who deals individuals with psychiatric and neurological problems here in XXXXXXX

In regard to your query it is almost clear that there is neurological problem rather than anything else. The presence of pain through out the right side of body indicates that there is some problem in left side of brain. The presence of significant weight loss along with other symptoms indicates possibility of tubercular meningitis which is very common in South Asian countries.

The location mentioned by you shows the United States but by the name it appears to me that you belong to some South Asian countries, particularly XXXXXXX Tubercular infections are rampant in this part of world and it is possible that you contacted this infection while visiting back home.

Hello Doctor,

I have not visited XXXXXXX since the day I came to US back in 2015. I was fit and fine then, but over couple of years things changed drastically. I am very frustrated on what is going on with me with this constant headache and weight loss. I am always fatigue, exhausted, tired so not sure what is right for me. I cannot concentrate on anything because of all the pain. It is affecting my daily life as well a lot. Please advise. Sometime I get pelvic pain whenever it comes my head also starts paining. Yesterday night the pain radiated to the middle part of the left side of my head. I am confused what can cause pain in entire right side of the body, why is this weight loss, I feel like there are some nerve that is very painful, it is very tough to explain.

I want to be cured asap, please advise if there is anything to be worried about. Brain is the most complex and very significant part of the body so I am very scared and get sleepless nights too. Please advise.

Secondly from the available description it appears that there is evidence of depression as many of the symptoms on current date can be explained on basis of depression. Considering this fact I feel there is need for simultaneous psychiatrist opinion so the things can be managed appropriately.

Unfortunately we do not have any alternative in case of infective causes of brain other than the biopsy when the diagnosis is in question.

I understand that any form of surgery is scary but the complication rate with brain biopsy is very low in compare to brain surgeries.
